KEY FUNCTION:
Provide best practice assessment, treatment and education services individually and in groups to adults and families with concurrent mental health and addiction concerns within a community mental health setting. Provide treatment for addictions, mood and anxiety disorders, axis 2 disorders and post-traumatic stress disorder (PTSD).

 

REPORTING:
Under the general direction of the Clinical Manager, Counselling & Treatment.

 

DUTIES:
1. Provide evidence-based treatment for addictions, concurrent disorders, mood disorders, anxiety disorders, PTSD, and axis 2 disorders.
2. Complete intake assessments and screening function for incoming referrals.
3. Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.
4. Document and chart assessments, intervention plans, progress notes, and outcomes in a timely manner.
5. Assist in and act as a resource for client and family education.
6. Consult and liaise with appropriate resources and stakeholders to ensure quality care and maintain patient safety.
7. Work collaboratively with community providers to provide wrap around care for complex clients.
8. Complete face-to-face crisis assessments for clients presenting at emergency sites.
9. Provide ongoing crisis management for clients waiting for addictions treatment.
10. Provide guidance, training and orientation for new staff and students.
11. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
13. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
14. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
15. Educate and promote health, safety and wellness in the work place.
16. Represent the department or program on various committees and in meetings as required.
17. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a four (4) year Bachelor’s Degree in Social Work from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
3. Certification in Cognitive Behavioral Therapy (CBT) including fourty (40) hours of theory, and supervision hours is required.
4. Certification in Non-Violent Crisis Intervention (NVCI) is required.
5. Additional training in Addictions and Counselling and Treatment is preferred.
6. Certification/training in Crisis Intervention skills is preferred.
7. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of two (2) years full-time equivalent experience (within the last 5 years) working in community mental health and addiction settings.
2. Experience treating addictions, mood and anxiety disorders with formal training and supervised practice in CBT is required.
3. Experience providing treatment in a group setting is preferred.
4. Experience treating axis 2 disorders, and providing treatment for PTSD is preferred.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated ability to practice in the areas of clinical counseling with adults and families. 2. Demonstrated health care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
3. Demonstrated skills in CBT, Dialectical Behavioral Therapy (DBT), and individual and group interventions.
4. Demonstrate strong client and family service orientation.
5. Demonstrated knowledge of culturally safe practices.
6. Demonstrated knowledge of evidence-based practices of recovery principles.
7. Demonstrated knowledge of community resources.
8. Demonstrated knowledge of the Mental Health Act and other relevant legislation.
9. Demonstrated excellent judgment and proven analytical skills.
10. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
11. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
12. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
13. Demonstrated superior interpersonal and communication skills, both written and verbal.
14. Demonstrated strong organizational skills with the ability to prioritize workload.
15. Demonstrated discretion and maturity when handling confidential information.
16. Demonstrated commitment to the safety of co-workers and patients.
