KEY FUNCTION:
Initiate and lead patient flow strategies across the organization in an effort to place the right patient in the right bed at the right time. Problem solve flow and occupancy pressures in a collaborative approach with internal and external stakeholders.
REPORTING:
Under the general direction of the Manager, System Transformation.
DUTIES:
- Apply HSN Bed Management Policies while following the guiding practices to track admitted patients through emergency, urgent and elective admissions in the most appropriate locations.
- Register and monitor all admission requests for region including isolation requirements and communicating with Infection Prevention and Control (IPAC) and the Emergency Department to ensure proper isolation is reflected on the admission order.
- Enter registration, transfers, admissions, discharges and information changes entry in Meditech.
- Communicate regularly with physicians’ office and staff from hospitals outside the region/country to acquire information for inpatient bed placement.
- Monitor and provide for data entry in the Provincial Hospital Resource System for Repatriations and Inter-facility transfers.
- Collaborate with units to problem solve bed issues and placement concerns; highlight challenges to the Patient Flow Supervisor.
- Participate in daily bed round meetings with directors, clinical managers, charge nurse and report on surgical demand bringing relevant information forward.
- Monitor and report the amount of telemetry packs available for the organization.
- Register patients by phone presenting to Labour and Delivery (i.e. inpatient registrations, labour triages and new births) in a timely manner.
- Ensure expired patients transition to the appropriate place with all required documentation following the Care Transitions Following Death procedure.
- Coordinate with other hospital departments (i.e. Environmental Services and Patient Transport) to ensure that patients are moved safely and efficiently.
- Monitor and report on bed utilization and occupancy rates to hospital management using system generated reports.
- Ensure that patient confidentiality and privacy are maintained at all times.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Participate in hospital training as required.
- Assist with improvement initiatives as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a graduate from a related post-secondary diploma program from an accredited college.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of six (6) months recent relevant experience.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of and proficiency in Medical Terminology.
- Proficiency in data entry and working knowledge of computerized patient information systems.
- Demonstrated computer skills with advanced skills and proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated ability to effectively interact and communicate with patients, families, and healthcare workers.
- Demonstrated interpersonal and communication skills.
- Ability to prioritize tasks.
- Demonstrated compliance with HSN’s privacy policy.
- Demonstrated commitment to the safety of co-workers and patients.
