KEY FUNCTION:
Provide leadership and direction through planning and organizing of the day-to-day operations of the Regional Outpatient Geriatric Medicine Service. Promote the delivery of optimal care/service to patients throughout the North East, through the management of human and material resources.

 

REPORTING:
Under the general direction of the Clinical Manager, Regional Outpatient Geriatric Medicine Service.

 

DUTIES:
1. Design, promote, and facilitate the use of systems to ensure that the provision of patient care is consistent with program and professional standards.
2. Identify and respond to problems and deficiencies in the delivery of optimal patient care and facilitate an inter-disciplinary approach to the formulation of solutions.
3. Support work schedules and staff assignments to ensure deliverables are met.
4. Coordinate and participate in clinical activities that require an interdisciplinary team.
5. Ensure program delivery is consistent with evidence-based and best practices.
6. Prepare and ensure timely communication/reports to all internal and external stakeholders (e.g. database management, statistical analysis and reporting to funding bodies).
7. Act as a resource to staff in client care matters and ensure patient/client needs are met in a safe and timely manner.
8. Act as liaison for external inquiries.
9. Support human resources which include: recruitment, selection and training.
10. Develop, review and revise policies, procedures, work standards, and guidelines to ensure excellent service delivery and adherence to administrative policies and procedures.
11. Assist with budget development and regular monitoring of financial outcomes.
12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
13. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
14. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
15. Educate and promote health, safety and wellness in the work place.
16. Represent the department or program on various committees and in meetings as required.
17. Perform other duties as required.

 

QUALIFICATIONS

 

EDUCATION AND TRAINING:
1. Minimum of a four (4) year Bachelor’s Degree with a specialization in a health-related field, from an accredited university.
2. A Master’s Degree from an accredited university is preferred.
3. Current Certificate of Registration in good standing with an applicable regulatory college is required.
4. Ministry of Labour “Worker Health and Safety Awareness” training certificate is required.

 

EXPERIENCE:
1. Minimum of five (5) years’ experience working as a Health Professional within a health care environment.
2. Minimum of two (2) years’ working in a leadership/supervisory role, preferably in a health care environment.
3. Minimum of five (5) years’ experience working within Specialized Geriatric Services.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated superior conflict management and resolution skills.
2. Proven ability to develop, monitor, administer and report on budgets.
3. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
4. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
5. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
6. Demonstrated superior interpersonal and communication skills, both written and verbal.
7. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
8. Demonstrated discretion and maturity when handling confidential information.
9. Demonstrated commitment to the safety of co-workers and patients.
