Health Sciences North (HSN) is seeking an innovative individual for the position of Dietitian to assess patient’s complex nutrition needs and participate in the development, implementations and evaluation of a comprehensive, interdisciplinary care plan.  The position will be specifically in the Diabetes Education program. The focus of the role will be providing individual and group appointments to patients living with diabetes and/or gestational diabetes within an interdisciplinary team.  The team includes Nurses, Social Worker, Pharmacist, Chiropodists, Physiotherapists, Nurse Practitioners, Endocrinologists and other Dietitians.  

 

The ideal candidate will be registered with the College of Dietitians of Ontario. We are looking for someone with a completion of a Bachelor’s Degree in Nutrition and Dietetics as well as an accredited Dietetic Internship Program completion.

Purpose:

Assess patient’s complex nutrition needs and participate in the development, implementation and evaluation of a comprehensive, interdisciplinary care plan. Provide therapeutic nutritional instruction while coordinating multiple dietary restrictions. Promote self-care management of diabetes and co-morbidities.  

 

Education and Training:

  • Proven registration with the College of Dietitians of Ontario is essential
  • Completion of a Bachelor’s Degree in Nutrition and Dietetics and an accredited Dietetic Internship Program is essential
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required

 

Experience:

  • Experience with patients with diabetes and its co-morbidities
  • Experience in an outpatient setting
  • Experience in individual and group counseling

 

Knowledge/ Skills/Ability:

  • Demonstrated knowledge and proficiency with Microsoft Suite Software, i.e Excel, Word, Outlook and PowerPoint
  • Demonstrated knowledge and proficiency with Meditech i.e. EMR, Patient Care System (electronic documentation), Community Wide Scheduling
  • Demonstrated ability to manage nutritional issues affecting patients with diabetes is essential

 

Personal Suitability:

  • Demonstrated positive work record and good attendance record
  • Demonstrated ability to work as a member of an Interprofessional team, as well as being able to work independently and prioritize workload is essential
  • Clear criminal record check is required
  • Valid Driver’s license, access to a vehicle and ability to travel throughout Greater Sudbury to provide outreach
  • Willingness to provide diabetes focused visits including hypertension management  (including blood pressure testing) and diabetes foot screening (including foot exams)
