KEY FUNCTION

To provide comprehensive nutritional assessments and care to complex medical and oncology patients.

 

REPORTING

Under the general direction of the Clinical Manager, Oncology, Medical and Palliative Unit.

 

DUTIES:

  1. Interpret anthropometric, biochemical, physiological data in relation to nutrition status.
  2. Calculate patient’s daily nutrition requirements (macro- and micro-nutrients including energy, protein, vitamins, minerals, fluids, etc.) based on available data.
  3. Provide Enteral and Parenteral Nutrition support, lab interpretation and correction.
  4. Provide therapeutic counselling to patients and/or family/caregiver(s) on an individual basis.
  5. Monitor patient progress and liaise with the multidisciplinary treatment team and other relevant health agencies (i.e. Home and Community Care).
  6. Complete nutritional supplement prescriptions as required.
  7. Attend clinical meetings as required.
  8. Identify current and potential nutrition related problems and develop and/or revise clinical nutrition policies and procedures.
  9. Develop and revise nutritional assessment and charting forms as required.
  10. Provide education and awareness on the nutritional management of medical issues and healthy eating.
  11. Develop and maintain an inventory of relevant patient education materials reflecting current scientific evidence and best practice standards.
  12. Attend clinical nutrition and other pertinent education opportunities (i.e. conferences, workshops), as required.
  13. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  14. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
  15. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  16. Educate and promote health, safety and wellness in the work place.
  17. Represent the department or program on various committees and in meetings as required. 
  18. Perform other duties as required. 

 

QUALIFICATIONS

EDUCATION AND TRAINING: 

  1. Minimum of a four (4) year Bachelor’s Degree in a Nutrition field, from an accredited university.
  2. Successful completion of an accredited Dietetic Internship program.
  3. Current Certificate of Registration in good standing with the College of Dietitians of Ontario.
  4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of two (2) years’ experience working in acute care.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated knowledge of enteral and parental nutrition.
  2. Demonstrated ability to work in an interdisciplinary setting and as a member of a multidisciplinary team.
  3. Demonstrated training, experience or utilization of lean methodology for process improvement.
  4. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  5. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  6. Demonstrated superior interpersonal and communication skills, both written and verbal.
  7. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  8. Demonstrated discretion and maturity when handling confidential information.
  9. Demonstrated commitment to the safety of co-workers and patients.
