KEY FUNCTION

Work collaboratively with a multidisciplinary team to provide safe and effective therapeutic exercise treatment and education for clients, while maintaining a functional capacity and level of independence. Assist in the development, provision, and evaluation of treatment provided in both individualized and group formats.

 

REPORTING

Under the general direction of the Clinical Manager, Pediatric Outpatient Services.

 

DUTIES

  1. Develop and implement recommendations to facilitate activities of normal living; encourage clients with special needs and circumstances to acquire new skills and get involved in health promotion leisure activities.
  2. Supervise therapeutic exercise sessions and offer vocational rehabilitation counselling.
  3. Observe, analyze, and record patients’ participation, reactions, and progress made during treatment sessions; modify treatment programs as needed.
  4. Design and supervise exercise sessions, develop exercise prescriptions based on the results of a graded exercise test, and participate in emergency responses.
  5. Develop a treatment plan to meet the needs of patients and their families based on needs assessment, patient interest, and objectives of therapy.
  6. Provide initial and follow-up assessments, goal setting, and motivational support to clients and families regarding long-term behavioural modification.
  7. Document within an electronic Patient Care System (PCS).
  8. Refer clients to other members of the interdisciplinary team, and participate in case conferences, team huddles, and team meetings.
  9. Participate in departmental research projects and maintain workload measurement statistics.
  10. Design and deliver group education sessions both in a classroom setting and via videoconference.
  11. Liaise and work with other agencies and physicians’ offices to coordinate patient care when required.
  12. Act as a Kinesiology preceptor for post-secondary students.
  13. Educate and promote health, safety and wellness in the work place.
  14. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  15. Determine and align improvement projects with HSN’s Strategic plan; monitor and adjust to achieve goal outcomes.
  16. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  17. Represent the department or program on various committees and in meetings as required. 
  18. Perform other duties as required. 

 

QUALIFICATIONS

EDUCATION AND TRAINING: 

  1. Minimum of a four (4) year Bachelor’s Degree in Kinesiology or Human Kinetics with a specialization in Health Promotion, from an accredited university.
  2. Current Certificate of Registration with the Ontario Kinesiology Association (OKA).
  3. Certificate in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED).
  4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of one (1) year full-time experience in exercise interventions with patients within the last five (5) years.
  2. Minimum of one (1) year full-time experience in pediatric health care within the last five (5) years.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated ability to actively participate in exercise leadership.
  2. Demonstrated knowledge and understanding of the Ontario Hospital Association (OHA) and Ontario Medical Association’s (OMA) Communicable Disease Surveillance Protocols.
  3. Demonstrated knowledge of literature and research about Type II Pediatric Diabetes and childhood obesity, and the co-morbidities related to each.
  4. Demonstrated organizational and prioritization skills.
  5. Demonstrated excellent judgment and proven analytical skills.
  6. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  7. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  8. Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  9. Demonstrated superior interpersonal and communication skills, both written and verbal.
  10. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
  11. Demonstrated discretion and maturity when handling confidential information.
  12. Demonstrated commitment to the safety of co-workers and patients.

 

PERSONAL SUITABILITY: 

  1. Successful criminal reference check is required for vulnerable sector.
  2. Demonstrated ability to work effectively as a member of an interdisciplinary team.
  3. Demonstrated ability to perform with minimal supervision; to prioritize duties.
