KEY FUNCTION

Provide safe and effective therapeutic exercise treatment and education for patients with chronic lung and cardiac disease, and maintain functional capacity and level of independence.

 

REPORTING

Under the general direction of the Clinical Manager, Integrated Chronic Pain Program, OBIRS, CDSMP, HAVEN/Hemophilia, Cardiac & Pulmonary Rehabilitation.

 

DUTIES

  1. Oversee therapeutic exercise sessions for outpatients of the Cardiac Pulmonary Rehabilitation department and offer vocational rehabilitation counselling.
  2. Design and oversee exercise sessions, develop exercise prescriptions based on the results of a graded exercise test, and participate in emergency responses.
  3. Provide initial and follow-up assessments, goal setting, and motivational support to clients and families regarding long-term behavioural modification.
  4. Document within an electronic Patient Care System (PCS).
  5. Act as a Kinesiology preceptor for post-secondary students, and design and deliver group education sessions in a classroom setting and via videoconference.
  6. Participate in departmental research projects and maintain workload measurement statistics.
  7. Refer patients to other members of the interdisciplinary team, and participate in case conferences, team huddles, and team meetings.
  8. Liaise and work with other agencies and physicians’ offices to coordinate patient care as required.
  9. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  10. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  11. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  12. Educate and promote health, safety and wellness in the work place.
  13. Represent the department or program on various committees and in meetings as required. 
  14. Perform other duties as required. 

 

QUALIFICATIONS

 

EDUCATION AND TRAINING: 

  1. Minimum of a four (4) year Bachelor’s Degree in Kinesiology or Human Kinetics, from an accredited university.
  2. Current Certificate of Registration within the Ontario Kinesiology Association (OKA).
  3. Current Certificate as a Certified Respiratory Educator (CRE) through the Canadian Network for Respiratory Care, or willing to obtain within two (2) years.
  4. Current Certificate as a Clinical Exercise Specialist (CEP) through the American College of Sports Medicine (ACSM) or willing to obtain within two (2) years.
  5. Current Certificate in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED).
  6. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of one (1) year full-time equivalent experience working in exercise interventions with patients within the last 5 years.
  2. Minimum of one (1) year full-time equivalent experience working in adult education within the last five (5) years.
  3. Minimum of two (2) years’ full-time equivalent experience in providing respiratory and/or cardiac care within the last five (5) years.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated ability to actively participate in exercise leadership, including hydrotherapy in a pool environment and resistance training.
  2. Demonstrated ability to work in an integrated team.
  3. Demonstrated planning, analytical, and problem-solving skills.
  4. Demonstrated training, experience or utilization of lean methodology for process improvement.
  5. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  6. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  7. Demonstrated superior interpersonal and communication skills, both written and verbal.
  8. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
  9. Demonstrated discretion and maturity when handling confidential information.
  10. Demonstrated commitment to the safety of co-workers and patients.
