KEY FUNCTION:
Work in collaboration with the mobile crisis rapid response team and crisis intervention services in order to provide transitional supports to individuals referred for service. Employ psychosocial rehabilitation approaches with a recovery focus. Develop and maintain a working and participatory partnership with the clients.
REPORTING:
Under the general direction of the Coordinator, Urgent Response and Care Transitions.
DUTIES:
1. Work with crisis team and other community mental health and justice partners to identify and engage eligible service recipients.
2. Work with the service client to mutually develop a short-term, individualized service plan that focuses on crisis stabilization and prevention and reflects client’s stated goals and needs.
3. In collaboration with individuals and community partners, develop a personalized transition plan with timelines.
4. Assist service recipients to achieve/maintain optimum health and wellness.
5. Refer to and access client’s desired/needed community and primary health care services (e.g. General Practitioner, housing, peer support, and counselling).
6. Assist in client’s development of self-management strategies (e.g. crisis avoidance, crisis planning, anger management, goal setting, time management, conflict resolution, and problem solving).
7. Assist in the development/fortification of client’s personal support networks.
8. Provide support and education to caregivers and/or the client’s personal support network as required.
9. Support client’s positive health goals (e.g. medication routines, nutrition, and physical activity).
10. Facilitate connections to work/school, training, and volunteering to encourage a sense of purpose and self-worth.
11. Provide encouragement, emotional support and motivation to service recipients.
12. Assist with the development/maintenance of client’s abilities relating to activities of daily living (e.g. budgeting, grocery shopping, housekeeping, cooking, and use of public transportation).
13. Assist in coordination of Community Treatment Orders (CTO) as required.
14. Develop partnerships and/or service agreements with other agencies, community services or primary care providers to ensure continuity of service provision as required (e.g. addictions services, employment training, developmental services, and family support).
15. Advocate on behalf of the client for services that are accessible and relevant to their goals and needs.
16. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
17. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
18. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
19. Educate and promote health, safety and wellness in the work place.
20. Represent the department or program on various committees and in meetings as required.
21. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Current Certificate of Registration in good standing with the applicable regulatory college (e.g. College of Nurses of Ontario (CNO), College of Occupational Therapists of Ontario (COTO), Ontario College of Social Workers and Social Service Workers in conjunction with a Master of Social Work degree (MSW) (OCSWSSW), College of Psychologists of Ontario at the Psychological Associate level (CPO), or College of Registered Psychotherapists of Ontario (CRPO).
2. Unit-specific certification, training, and education may be required.
3. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of two (2) years full-time equivalent experience (within the last five (5) years) in case management and in a community mental health or addictions setting.
2. Minimum of five (5) years full-time equivalent experience working on an adult inpatient psychiatric unit or Crisis Intervention Program.
3. Experience working with people with mental health and addictions issues who have experienced conflicts with the law and exposure to the criminal justice system is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge and understanding of the Criminal justice system.
2. Demonstrated knowledge of Medical Terminology, Mental Health Act, Accessibility for Ontarians with Disabilities Act (AODA), the Ontario Human Rights Code, Personal Health Information Protection Act (PHIPA), and other relevant legislation.
3. Demonstrated ability to address clients’ primary needs quickly (e.g. income, Ontario Disability Support Program (ODSP), food, shelter, and primary care).
4. Demonstrated knowledge of trauma-informed care (e.g. understanding the impacts of physical, psychological and emotional trauma).
5. Demonstrated ability to educate others to support effective strategies for supporting those affected by trauma(s).
6. Demonstrated knowledge and ability to navigate Ontario’s mental health and addictions, developmental services and community support service sectors.
7. Demonstrated strong relationship-building skills to develop and reinforce clients’ social supports, connections to resources and events that foster social relationships, reduce loneliness, and leverage motivation to live fully.
8. Demonstrated practical comprehension of intersectional oppression; ability to understand and discuss how marginalization, discrimination, racism, misogyny, trauma, etc., affect individuals’ ability to live stably and fully.
9. Demonstrated strong assessment and organizational skills to develop a holistic service plan.
10. Demonstrated motivational interviewing skills to capitalize on client’s motivation.
11. Demonstrated excellent judgment and proven analytical skills.
12. Demonstrated ability to mentor students and support personnel.
13. Demonstrated knowledge of current trends in general health care and community resources.
14. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
15. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
16. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
17. Demonstrated superior interpersonal and communication skills, both written and verbal.
18. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
19. Demonstrated discretion and maturity when handling confidential information.
20. Demonstrated commitment to the safety of co-workers and patients.
