KEY FUNCTION:
Assist in establishing and maintaining a safe and therapeutic environment through the provision of therapeutic intervention and engagement with the patients and the inter-professional team. Provide care to patients in acute care transitioning to the Reactivation Care Centre.

 

REPORTING:
Under the general direction of the Clinical Manager, Acute and Reactivation Care Unit.

 

DUTIES:
1. Implement therapeutic, geriatric-focused care of patients with the goal of identifying restorative potential.
2. Work collaboratively with the clinical team to best meet the unit or patient’s needs.
3. Implement and guide patients through recommended treatment and activity programs both individually and in groups (e.g. assist with preparing patients for treatment, treatment modalities, activities of daily living, walking programs, transfers, and communication skills).
4. Instruct the patient in the use of equipment and adaptive devices as directed by the therapist.
5. Assist with patient referrals and/or patient scheduling in accordance with unit practices under the direction of the therapist.
6. Maintain a safe environment by caring for equipment breakdowns, remove hazards, complete maintenance requisitions and notify therapist.
7. Document patient intervention in the health record as per unit requirements.
8. Report observation of patient’s behaviour, progress and concerns to the assigning therapist.
9. Work collaboratively with the clinical team to best meet the unit or patient’s needs.
10. Provide care during transition from acute care to the Reactivation Care and additional care destination outside of HSN, including partnership with internal and community resources to maximize successful transitions.
11. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
13. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
14. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
15. Educate and promote health, safety and wellness in the work place.
16. Represent the department or program on various committees and in meetings as required.
17. Perform other duties as required.

 

QUALIFICATIONS

 

EDUCATION AND TRAINING:
1. Minimum of a two (2) year Diploma in Occupational Therapist Assistant (OTA), Physiotherapist Assistant (PTA), or a Rehabilitation Assistant from an accredited college.
2. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of two (2) years’ experience working within a health care environment.
2. Recent experience working with older adults is required.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of community support services, home and community care, rehabilitative services, adult day programs, and other social services.
2. Demonstrated ability to assist in the development and maintenance of a therapeutic environment.
3. Demonstrated ability to instruct and guide patients through recommended treatment and activity programs.
4. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
5. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
6. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
7. Demonstrated superior interpersonal and communication skills, both written and verbal.
8. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
9. Demonstrated discretion and maturity when handling confidential information.
10. Demonstrated commitment to the safety of co-workers and patients.
