Purpose:

Provide specialized comprehensive geriatric assessment, analysis and mental health treatment for older adults living in the community with complex mental health needs and geriatric syndromes.  Provide outreach visits to individuals in their home environment, including community, retirement, and long term care facilities across Manitoulin.

 

Standard Qualifications:

  • A graduate from an approved School of Nursing (university or community college) with a current registration in good standing with the College of Nurses of Ontario (CNO).

 

Required Qualifications:

  • Successful completion of the Geriatric Assessor Training course or equivalent is required.
  • Core Indigenous Cultural Competency Training, Gentle Persuasive Approach (GPA) and the P.I.E.C.E.S Training program are required.
  • Current Basic Cardiac Life Support (BCLS) certificate is required.
  • Current certification in Automated External Defibrillator (AED) is required.
  • Ministry of Labour “Worker Health and Safety awareness in 4 steps” training certificate is required.
  • Successful Criminal Record check with vulnerable sector.
  • Canadian Nursing Association (CNA) Certification in Gerontological Nursing is an asset.

 

Experience:

  • Minimum of three (3) years of full-time equivalent experience within the last five (5) years providing Geriatric Nursing in a specialized geriatric setting.
  • Recent experience completing comprehensive mental health assessment and treatment in a community setting.

 

General Skills:

  • Demonstrated ability to travel throughout Manitoulin Island and provide outreach to assess and treat individuals in their homes
  • Demonstrated skills conducting comprehensive geriatric assessment, analyzing, developing and adminstering treatment plans based on the comprehensive geriatric assessment.
  • Demonstrated knowledge of geriatric mental health best practice tools and interventions, including Comprehensive Geriatric Assessment (CGA), Cognitive Behavioural Therapy, RAID, CASE, SAST Mini Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Barthel Index, Confusion Assessment Method (CAM), Clock Drawing Test, Phonemic/Semantic Fluency, Mini-cog, Trails Making Test, Geriatric Depression Scale, and Cornell Scale for Depression in Dementia.
  • Demonstrated skills inthe management of geriatric mental health syndromes, including depression, anxiety, delirium, substance misuse, risk of harm to self or others, social isolation and other age-related mental health disorders.
  • Demonstrated knowledge of Indigenous culture, local Indigenous community groups, and resources.
  • Demonstrated knowledge of chronic disease management in the older population.
  • Demonstrated positive interpersonal relations and patient-teaching skills.
  • Demonstrated commitment to patient and family-centered care.
  • Ability to interact with physicians and other healthcare providers in a collaborative, professional, intelligent, and effective manner.
  • Demonstrated ability to effectively make decisions, deploy critical thinking and other skills required to organize/prioritize tasks.
  • Demonstrated strong therapeutic communication skills and ability to communicate both verbally and in writing in a clear, concise manner, considering the intended audience.
  • Demonstrated ability to effectively make decisions, deploy critical thinking and other skills required to organize/prioritize tasks.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of co-workers and patients.
