KEY FUNCTION:
Perform triage and screen referrals received for the Ontario Structured Psychotherapy Program. Review referrals for completeness and determine eligibility in the program; send referrals to a therapist for an intake assessment to confirm their eligibility into the program. Provide individual and/or group centered evidence-based, short-term, Cognitive Behavioural Therapy and related approaches to adults who experience mild to moderate depression, anxiety, and anxiety-related conditions.

 

REPORTING:
Under the general direction of the Clinical Manager, Ontario Structured Psychotherapy Program.

 

DUTIES:
1. Provide triage and intake to clients seeking Ontario Structured Psychotherapy (OSP) services.
2. Conduct clinical assessments (e.g. Diagnostic Assessment Research Tool, Patient Health Questionnaire-9 (PHQ-9), General Anxiety Disorder-7 (GAD-7)) to identify symptoms experienced by clients and to create individualized treatment plans.
3. Provide individual and group protocol-based structured psychotherapy, including Cognitive Behavioural Therapy (CBT) and Cognitive Processing Therapy (CPT), in adherence with the OSP Clinical Model and Training guidelines to clients with mood, anxiety, concurrent disorders and trauma experiences.
4. Provide psychotherapy services virtually and in-person based on client and program needs.
5. Participate in the OSP training program, consultation model, and ongoing education.
6. Participate in regular clinical consultation and related activities in adherence with fidelity monitoring requirements and program guidelines.
7. Maintain accurate and timely documentation and reports in adherence with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and program specific requirements.
8. Collaborate with team members across the OSP program’s network of organizations under the provision of care.
9. Contribute to ongoing program development, program evaluation and implement new practices as required.
10. Assist in providing service navigation for clients whose needs are beyond the offerings of the OSP program.
11. Work in partnership with clients, their formal and informal supports and the team to support each clients’ successful integration into their communities.
12. Contribute to program development, research and educational activities of the service.
13. Maintain up-to-date charting/records in compliance with HSN policies and OSP procedures, and mandatory reporting requirements and standards.
14. Represent the department or program on various committees and in meetings as required.
15. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
16. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
17. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
18. Educate and promote health, safety and wellness in the work place.
19. Represent the department or program on various committees and in meetings as required.
20. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a four (4) year Bachelor’s Degree in Social Work, from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
3. Non-Violent Crisis Intervention training (NVCI) is required.
4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of one (1) year experience in triage, clinical assessment, and treatment delivery of evidence-based CBT and CPT.
2. Experience providing mental health and addiction screening and intake with ability to make clinical recommendations is required.
3. Experience with in-person and virtual care delivery is preferred.
4. Experience working in a community-based or outpatient setting is preferred.
5. Experience utilizing evidence-based CBT treatment models with clients experiencing symptoms of depression, anxiety, and anxiety related conditions is preferred.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated ability to assess client symptoms and needs, and triage client to the appropriate OSP service across the stepped care pathway or to another service.
2. Demonstrated knowledge and practiced skills in providing culturally safe and responsive mental health services to clients of diverse ethno-racial and cultural backgrounds.
3. Demonstrated knowledge of DSM-V and psychosocial issues affecting people living with mental health and addiction issues.
4. Demonstrated knowledge of the science and practice of structured psychotherapy including a full range of psychotherapeutic and assessment techniques to assess, plan, coordinate and implement clinical protocols.
5. Demonstrated knowledge of CBT, CPT, and other evidence-based structured psychotherapies; experience working with people with depression and/or anxiety (e.g. obsessive-compulsive concerns and post-traumatic stress).
6. Demonstrated ability to distinguish features of depression, anxiety, anxiety related conditions, and the etiology/treatment of these disorders.
7. Demonstrated critical thinking and problem-solving skills.
8. Demonstrated knowledge of relevant legislation (e.g. Mental Health Act, Personal Health Information Protection Act, Regulated Health Professions Act, Health Care Consent Act, professional standards to ensure compliance and consistency in provision of client care).
9. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
10. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
11. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook), patient information systems, and other program specific tools.
12. Demonstrated superior interpersonal and communication skills, both written and verbal.
13. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
14. Demonstrated discretion and maturity when handling confidential information.
15. Demonstrated commitment to the safety of co-workers and patients.
