KEY FUNCTION:
Provide best practice assessment and treatment individually and in groups to adults and families within a community mental health setting. Provide treatment for mood and anxiety disorders, axis 2 disorders and Post-Traumatic Stress Disorder (PTSD).

 

REPORTING:
Under the general direction of the Clinical Manager, Counselling and Treatment and Rural Programs.

 

DUTIES:
1. Provide evidence-based treatment for mood disorders, anxiety disorders, PTSD, and axis 2 disorders.
2. Complete intake assessments.
3. Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.
4. Document assessments, treatment plans, progress notes, and outcomes in a timely manner.
5. Assist in and act as a resource for client and family education.
6. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
7. Provide guidance, training and orientation for new staff and students.
8. Work collaboratively with community providers to provide wrap around care for complex clients.
9. Educate and promote health, safety and wellness in the work place.
10. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
11. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
12. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
13. Represent the department or program on various committees and in meetings as required.
14. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a four (4) year Bachelor’s Degree in Social Work, from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
3. Certification in Cognitive Behavioral Therapy including fourty (40) hours of theory, and supervision hours.
4. Additional training in Addictions and Counselling and Treatment is preferred.
5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of five (5) years’ experience working in a mental health setting.
2. Experience in community-based, out-patient programs is required.
3. Experience treating mood and anxiety disorders with formal training and supervised practice in Cognitive Behavioral Therapy is preferred.
4. Experience providing treatment in a group setting is preferred.
5. Experience treating axis 2 disorders, and providing evidence-based treatment for PTSD preferred.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated ability to practice in the areas of Clinical counseling with adults and families. 2. Demonstrated Health Care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
3. Demonstrated skills in Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and individual and group interventions.
4. Demonstrated excellent judgment and proven analytical skills.
5. Demonstrate strong client and family service orientation.
6. Demonstrated knowledge of culturally safe practices.
7. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
8. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
9. Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
10. Demonstrated superior interpersonal and communication skills, both written and verbal.
11. Effective time management and organizational skills with the ability to organize and prioritize as required.
12. Demonstrated discretion and maturity when handling confidential information.
13. Demonstrated commitment to the safety of co-workers and patients.

 

PERSONAL SUITABILITY:
1. Ability to use tact and discretion in dealing with health care providers and patients.
2. Successful criminal reference check is required for vulnerable sector.
3. Demonstrated ability to work effectively as a member of an interdisciplinary team.
4. Valid driver’s license with access to reliable vehicle.
5. Demonstrated commitment to ongoing professional development.
6. Demonstrated professionalism in dealing with confidential and sensitive issues.
7. Demonstrated positive work record and good attendance record.
8. Ability to meet the physical and sensory demands of the job.
9. Ability to travel between local sites.
