KEY FUNCTION:   

Provide best practice assessment, treatment, navigation and support to individuals and families with mental health, substance use and/or concurrent disorders across the spectrum (e.g. inpatient, community).

 

REPORTING:     

Under the general direction of the Clinical Manager, Inpatient Addictions Medicine.

 

DUTIES:

  1. Work in collaboration with the multidisciplinary team to provide psychosocial support to patients admitted to the Addictions Medicine Unit.
  2. Liaise with Acute Inpatient Psychiatry and applicable medical units to obtain information regarding care planning and ensure the continuity of care from an inpatient to outpatient setting including medical assessment and treatment as required.
  3. Liaise with Addictions Medicine Consult Team (AMCT), Rapid Access Addictions Medicine (RAAM) clinic,   Outpatient Addictions and Gambling Service (OAGS) and any relevant HSN or community agencies to support the continuation of care or service needs when individuals discharge from hospital.
  4. Complete the development and implementation of treatment plans with clients and families accessing services.
  5. Complete addictions assessments for treatment planning purposes with individuals as required (i.e. Global Appraisal of Individual Needs Short Screener (GAINs SS), Global Appraisal of Individual Needs Q3 (GAINs Q3), etc.)
  6. Document assessments, intervention plans, progress notes and outcomes in a timely manner as per program policy standards.
  7. Assist in and act as a resource for client and family education.
  8. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
  9. Provide guidance, training and orientation for new staff and students.
  10. Work collaboratively with community providers to provide wrap around care for complex clients.
  11. Establish and maintain pathways to refer clients to on-going services post hospital stay.
  12. Facilitate a variety of education sessions, group treatment/counselling sessions, in collaboration with Addictions Workers and Peer Support staff.
  13. Use digital technology to complete off site assessments and/or sessions as required.
  14. Practice from a harm reduction approach when providing health teaching to clients and families.
  15. Apply the Code of Ethics and Standards of Practice as outlined by the Ontario College of Social Workers and Social Service Workers.
  16. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  17. Participate in the accreditation process by engaging in best practices, audits, assessments and improvement initiatives.
  18. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  19. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  20. Educate and promote health, safety and wellness in the workplace.
  21. Represent the department or program on various committees and in meetings as required. 
  22. Perform other duties as required. 

 

 

QUALIFICATIONS

 

EDUCATION AND TRAINING: 

  1. Minimum of a Bachelor’s Degree in Social Work from an accredited university. 
  2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
  3. Current Certification in Staged Screening and Assessment (SSA) is required.
  4. Current Certification in Harm Reduction is preferred.
  5. Certifications in one or more structured psychotherapy modality (e.g., Cognitive Behavioural Therapy (CBT), Motivational Interviewing (MI), Dialectical Behavioural Therapy (DBT)) are preferred.
  6. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

 

EXPERIENCE: 

  1. Minimum of three (3) years’ full-time equivalent experience working in a Mental Health and Addictions setting.
  2. Minimum of two (2) years’ full-time equivalent experience within the last five (5) years’ working in an acute medical setting.
  3. Experience in community-based, out-patient programs is required.
  4. Experience completing Staged Screening and Assessment (SSA) is required.
  5. Experience providing individual and group setting treatment is required.
  6. Experience in program development and providing education is preferred.

 

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated knowledge of addictions related care (i.e. withdrawal symptoms, withdrawal management, and medication treatment available).
  2. Demonstrated ability to utilize catalyst/Drug and Alcohol Treatment Information System (DATIS) and B-care system for reporting/documentation.
  3. Demonstrated knowledge of evidence based practices of recovery principles and stages of change.
  4. Demonstrated ability to complete addictions assessments, treatment planning, and referrals.
  5. Demonstrated ability to practice in the areas of clinical counseling with adults and families; strong client and family service orientation.
  6. Demonstrated health care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
  7. Experience providing both individual and group interventions.
  8. Demonstrated excellent judgment, critical thinking and proven analytical skills. 
  9. Demonstrated knowledge of how to apply harm reduction practices.
  10. Demonstrated knowledge of culturally safe practices.
  11. Demonstrated training, experience or utilization of lean methodology for process improvement.
  12. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  13. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  14. Demonstrated compliance with HSN’s privacy policy.
  15. Demonstrated superior interpersonal and communication skills, both written and verbal.
  16. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  17. Demonstrated discretion and maturity when handling confidential information.
  18. Demonstrated commitment to the safety of co-workers and patients.
