Social Worker Masters (Outpatient Neuro Rehab)

KEY FUNCTION: 

Provide rehabilitation programming with a focus on neurological rehabilitation for outpatients post-discharge from hospital. Responsible for the provision of services including assessment, referral, short-term intervention, locating and arranging resources, and client/family education.

REPORTING:      

Under the general direction of the Clinical Manager, Intensive Rehabilitation Unit.

DUTIES:

1. Participate in a pilot service for outpatient stroke and neurological patients.
2. Support implementation and adoption of CorHealth Ontario’s Model of Care for Community Stroke Rehabilitation.
3. Participate in the intake and/or transitonal care processes for referred patients from hospital.
4. Provide individual and group therapies to clients and families, including the provision of support with respect to acceptance and loss, psycho-education, and skill building in areas such as relaxation, stress management, problem solving, sleep hygiene, and self-advocacy.  
5. Communicate pertinent information and recommendations to clients/care providers and provide education.
6. Conduct home visits as required.
7. Participate in or support a virtual model of therapeutic care as required.
8. Maintain up-to-date charting/records in compliance with HSN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
9. Participate in developing, planning, and implementing new therapeutic programs based on clinical needs.
10. Participate in research, program development and evaluation, and quality assurance activities.
11. Actively pursue learning opportunities and communicate new developments and best practices through education sessions with team members.
12. Develop an intervention plan in collaboration with patients, families, stakeholders, and the interdisciplinary team, based on identified needs.
13. Document intervention plans according to specific HSN and professional standards.
14. Assist in overcoming barriers within the health care system to ensure timely delivery of services.
15. Help patients identify and use health resources, including those available in the community and through agencies.
16. Liaise with community agencies and assist with making connections to community resources/supports as required.
17. Provide short-term counselling services, with particular regard to individual/family issues and decisions that impact the health of the patient/family.
18. Balance group, same day, urgent, and individual counselling.
19. Collaborate with members of an interdisciplinary health team to plan, implement, coordinate, and evaluate patient care in consultation with patients and families.
20. Participate in the service’s discharge planning process.
21. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
22. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
23. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
24. Educate and promote health, safety and wellness in the work place.
25. Represent the department or program on various committees and in meetings as required.
26. Perform other duties as required. 

QUALIFICATIONS

EDUCATION AND TRAINING: 

1. Minimum of a Master’s Degree in Social Work from an accredited University.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
3. Certification in Cognitive Behavioural Therapy (CBT), Motivational Interviewing (MI), and Mindfulness-based stress reduction (MBSR) is preferred.
4. Completion of the CorHealth’s Interprofessional Stroke Core Competency Framework or willingness to complete within one (1) month of employment is required.
5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

EXPERIENCE: 

1. Experience working with patients who have had a stroke or other neurological and chronic conditions or muskuloskeletal disorders, in a rehabilitation or community setting is preferred.
2. Clinical experience within a health care environment with a focus on Mental Health is preferred.
3. Experience in group and individual counselling is required.

6. Demonstrated ability to mentor students and support personnel.
7. Demonstrated knowledge of Medical Terminology.
8. Demonstrated excellent judgment and proven analytical skills.
9. Demonstrated training, experience or utilization of lean methodology for process improvement.
10. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
11. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
12. Demonstrated superior interpersonal and communication skills, both written and verbal.
13. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
14. Demonstrated discretion and maturity when handling confidential information.
15. Demonstrated commitment to the safety of co-workers and patients.
