KEY FUNCTION

Responsible for the provision of services including assessment, referral, short-term intervention, locating and arranging resources, and education for clients of the Chronic Disease Self-Management, OBIRS and Integrated Chronic Pain Program. Promote and foster a culture in which the staff manage chronic pain patients and their families with understanding and compassion.

 

REPORTING 

Under the general direction of the Clinical Manager, Chronic Disease Self-Management, OBIRS and Integrated Chronic Pain, Haven/ Hemophilia, Cardiac and Pulmonary.

 

DUTIES:

  1. Participate in intake assessments and attend client case conferences as required.
  2. Provide individual and group therapies to clients and families, including the provision of support with respect to acceptance and loss, psycho-education, and skill building in areas such as relaxation, stress management, problem solving, sleep hygiene, and assertiveness.
  3. Communicate pertinent information and recommendations to clients/care providers and provide education regarding treatment strategies as required.
  4. Participate in assessments within the regional catchment area by utilizing Telemedicine.
  5. Maintain up-to-date charting/records in compliance with HSN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
  6. Participate in developing, planning, and implementing new therapeutic programs based on clinical needs.
  7. Participate in research, program development & evaluation, and quality assurance activities.
  8. Actively pursue learning opportunities and communicate new developments and best practices through education sessions with team members.
  9. Develop an intervention plan in collaboration with patients, families, stakeholders, and the interdisciplinary team, based on identified needs.
  10. Document intervention plans according to specific HSN and professional standards.
  11. Assist in overcoming barriers within the health care system to ensure timely delivery of services.
  12. Help patients identify and use health resources, including those available in the community and through agencies.
  13. Liaise with community agencies and assist with making connections to community resources/supports as required.
  14. Provide short-term counselling services, with particular regard to individual/family issues and decisions that impact the health of the patient/family.
  15. Balance group, same day, urgent, and individual counselling.
  16. Collaborate with members of an interdisciplinary health team to plan, implement, coordinate, and evaluate patient care in consultation with patients and families.
  17. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  18. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  19. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  20. Educate and promote health, safety and wellness in the work place.
  21. Represent the department or program on various committees and in meetings as required. 
  22. Perform other duties as required. 

 

QUALIFICATIONS

 

EDUCATION AND TRAINING: 

  1. Minimum of a Master’s Degree in Social Work from an accredited University.
  2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
  3. Certification in Cognitive Behavioural Therapy (CBT), Motivational Interviewing (MI), and Mindfulness-based stress reduction (MBSR) is preferred.
  4. Successful completion of Non-violent Crisis Intervention course is required.
  5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of three (3) years’ relevant clinical experience in the field of Chronic Disease, preferably in a Chronic Pain Clinic and/or Rehabilitation department.
  2. Clinical experience in a Mental Health/Addictions department within a health care environment is preferred.
  3. Experience in assessment, treatment and counselling of patients with chronic disease.
  4. Experience in group and individual counselling.
  5. Experience in community-based, out-patient/regional programs is preferred.

  4. Demonstrated knowledge of Indigenous and Francophone culture.
  5. Demonstrated knowledge and use of the Ontario Telemedicine Network (OTN; OTN-PCVC) and willingness to use in clinical practice.
  6. Demonstrated ability to mentor students and support personnel.
  7. Demonstrated discretion and maturity when handling confidential information.
  8. Demonstrated knowledge of Medical Terminology.
  9. Demonstrated excellent judgment and proven analytical skills.
  10. Demonstrated training, experience or utilization of lean methodology for process improvement.
  11. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  12. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  13. Demonstrated superior interpersonal and communication skills, both written and verbal.
  14. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  15. Demonstrated discretion and maturity when handling confidential information.
  16. Demonstrated commitment to the safety of co-workers and patients.
