KEY FUNCTION

Participate in the development and implementation of discharge plans for mental health and addictions patients.  Provide support, navigation, and referrals to community-based services and resources to support Alternative Level of Care (ALC) patients or potential ALC patients.

 

REPORTING

Under the general direction of the Coordinator, Urgent Response and Care Transitions.

            

          DUTIES

  1. Participate in the development of discharge planning for individuals that have presented with mental health and/or addictions related concern.
  2. Provide patient-centred discharge support to mental health and addictions patients; engage both patient and family and care partners as required.
  3. Implement discharge plan, including advocacy and referrals.
  4. Navigate patients towards appropriate community-based services/supports (e.g. income, housing, peer, vocational, legal services, physical healthcare, mental health/addictions services, counselling services, etc.).
  5. Work collaboratively with service providers such as Crisis Intervention, psychiatrists, Emergency Department hospital staff,  Acute Inpatient Psychiatry Emergency Department Liaison, Inpatient Unit hospital staff, Indigenous Navigator and Youth Navigator to support safe discharges/warm transitions from the Emergency Department or other designated units.
  6. Provide follow-up support to identified individuals; focus on identifying and resolving gaps/barriers resulting in re-presentation to hospital as required.
  7. Maintain clinical records ensuring compliance with professional standards of practice.
  8. Provide education to patients and families regarding available community services and resources.
  9. Provide appropriate individual and systemic advocacy in order to address individual/systemic barriers.
  10. Maintain knowledge of the Mental Health Act and other relevant legislation relevant in the provision of care.
  11. Ensure patient, visitor and staff safety by identifying and taking corrective actions; report any safety concerns to immediate supervisor.
  12. Evaluate patient care and satisfaction on an ongoing basis; formulate solutions for improvement.
  13. Provide guidance, training, orientation and preceptorship to new staff and/or students.
  14. Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
  15. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  16. Determine and align improvement projects with HSN‘s Strategic Plan; monitor and adjust to achieve program and organizational goals and outcomes.
  17. Educate and promote health, safety and wellness in the work place.
  18. Represent the department or program on various committees and in meetings as required. 
  19. Perform other duties as required. 

 

QUALIFICATIONS

EDUCATION AND TRAINING: 

  1. Minimum of a two (2) year diploma in a Health or Social Services related field, from an accredited college or equivalent.
  2. Graduate certificate in Community and Health Services Navigation is preferred.
  3. Non-Violent Crisis Intervention (NVCI) is required.
  4. Current Registration with a regulatory college preferred.
  5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of three (3) years’ experience working in a Mental Health and Addictions health care environment.
  2. Recent experience in program development preferred.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated superior knowledge of the Mental Health Act.
  2. Demonstrated ability to execute discharge plans and make connections to community service providers. 
  3. Demonstrated knowledge of existing community based programs and services.
  4. Demonstrated knowledge of housing programs within Greater Sudbury.
  5. Demonstrated training, experience or utilization of lean methodology for process improvement.
  6. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  7. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  8. Demonstrated superior interpersonal and communication skills, both written and verbal.
  9. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  10. Demonstrated discretion and maturity when handling confidential information.
  11. Demonstrated commitment to the safety of co-workers and patients.
