KEY FUNCTION:
Prepare charts for the department ensuring that required information is assembled for quality patient care.

 

REPORTING:
Under the general direction of the Manager, Bookings and Chart Preparation.

 

DUTIES:
1. Retrieve diagnostic reports, lab results, physician dictations, pathology reports, surgical reports, and discharge/hospital admission information from computer information systems.
2. Retrieve additional patient information from other hospitals, doctor’s offices, independent labs and healthcare facilities as required.
3. Collate all incoming information and ensure that reports are filed in the appropriate location within the correct patient chart.
4. Document chart deficiencies and communicate outstanding reports as required.
5. Perform chart reviews to determine required information for the following day’s visits.
6. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
7. Participate in hospital training as required.
8. Assist with improvement initiatives as required.
9. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a Grade 12 Diploma or equivalent (i.e. General Education Development (GED) or Academic and Career Entrance (ACE)).
2. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Three (3) years of recent related experience is preferred.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of Medical Terminology.
2. Demonstrated ability to type a minimum of 50 words per minute.
3. Demonstrated proficiency in data entry.
4. Demonstrated computer skills with advanced proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
5. Demonstrated ability to effectively interact and communicate with patients, families, and healthcare workers.
6. Demonstrated interpersonal and communication skills.
7. Demonstrated ability to prioritize tasks.
8. Demonstrated compliance with HSN’s privacy policy
9. Demonstrated commitment to the safety of co-workers and patients.
