KEY FUNCTION:
Work as a member of the Assertive Community Treatment Team (ACTT) in providing psychosocial rehabilitation interventions that support individuals with serious mental illness to live more independently in the community.
REPORTING:
Under the general direction of the Clinical Manager, Psychosocial Rehabilitation.
DUTIES:
- Participate in providing activity-based groups, take an active role in supporting clients as they re-enter vocational and educational settings, and support individuals in activities of daily living, self-care and medication adherence.
- Provide a range of individual and group therapies specific to the complex and ever-changing needs of the individuals referred to the ACTT, including evidence-based treatment for mood disorders, anxiety disorders, and axis 2 disorders.
- Consult and liaise with appropriate resources to ensure quality care, maintain patient safety, and manage suicidal risk as required.
- Work collaboratively with all team members to determine, implement, and evaluate support plans.
- Provide formal and informal teaching to university students and other staff as required.
- Select and provide guidance to students receiving field instruction, and liaise with related educational institutions.
- Maintain up-to-date charting/records in compliance with HSN policies and procedures and mandatory reporting requirements and standards.
- Complete daily statistical information.
- Educate and promote health, safety and wellness in the work place.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a four (4) year Bachelor’s Degree in Social Work or other mental health related field, from an accredited University.
- Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
- Non-Violent Crisis Intervention (NVCI) is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of three (3) years’ experience in providing community mental health services to those with serious mental illness.
- Experience in providing psychosocial rehabilitation interventions to individuals and groups.
- Experience supporting individuals with serious mental illness return to employment and/or vocational activities is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of rehabilitation and recovery principles.
- Demonstrated knowledge of community resources and supports.
- Demonstrated knowledge of the Mental Health Act and other relevant legislation.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software, e.g. Word, Excel, Power Point and Outlook and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Effective time management and organizational skills with the ability to organize and prioritize as required
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
PERSONAL SUITABILITY:
- Demonstrated ability to work effectively as a member of an interdisciplinary team.
- Ability to use tact and discretion in dealing with health care providers and patients.
- Successful criminal reference check is required for vulnerable sector.
- Demonstrated commitment to ongoing professional development.
- Demonstrated professionalism in dealing with confidential and sensitive issues.
- Demonstrated positive work record and good attendance record.
- Ability to meet the physical and sensory demands of the job.
- Ability to travel between local sites.
