KEY FUNCTION:

Provide comprehensive clinical nutrition assessments and care to caregivers and patients living with Diabetes. Collaborate as part of the interdisciplinary team to assist patients in meeting care plan goals.

 

REPORTING:

Under the general direction of the Coordinator, Diabetes, Bariatrics, Nutrition Counselling and Ophthalmology.

 

DUTIES:

  1. Assess patient’s complex nutrition needs and participate in the development, implementation and evaluation of a comprehensive, interdisciplinary care plan.
  2. Interpret anthropometric, biochemical, sociological and physiological data in relation to nutrition status.
  3. Calculate patient’s daily macro and micro-nutrition requirements (e.g. energy, protein, vitamins, minerals, fluids, etc.) based on available data.
  4. Provide therapeutic counselling to patients and/or family/caregiver(s) on an individual or group basis in relation to their nutrition and diabetes care plan goals
  5. Provide therapeutic nutritional instruction while coordinating multiple dietary restrictions.
  6. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  7. Attend clinical meetings as required.
  8. Identify current and potential nutrition related problems and develop and/or revise clinical nutrition policies and procedures.
  9. Act as a preceptor to dietetic learners from Northern Ontario Dietetic Internship Program (NODIP) / Northern Ontario School of Medicine (NOSM) University.
  10. Provide education and awareness on the nutritional management of medical issues and healthy eating.
  11. Develop and maintain an inventory of relevant patient education materials reflecting current scientific evidence and best practice standards.
  12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  13. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  14. Educate and promote health, safety and wellness in the work place.
  15. Represent the department or program on various committees and in meetings as required. 
  16. Perform other duties as required. 

 

QUALIFICATIONS

 

EDUCATION AND TRAINING:

  1. Minimum of a four (4) year Bachelor’s Degree in Nutrition and Dietetics, from an accredited university.
  2. Current Certificate of Registration in good standing with the College of Dietitians of Ontario.
  3. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:

  1. Minimum of two (2) years’ relevant clinical experience in the field of Chronic Disease, preferably in a Diabetes Care Services Program.
  2. Minimum of one (1) year of experience working in both an inpatient and outpatient clinical setting within a health care environment.
  3. Experience in assessment, treatment and counselling of patients with chronic disease is required.
  4. Experience with individual and group nutrition counselling is preferred.

 

KNOWLEDGE/SKILLS/ABILITIES:

  1. Demonstrated ability to manage various nutrition issues, counsel patients on evidence-based nutrition principles and recommend dietary modifications appropriate for the patient’s skills, beliefs, lifestyle and health concerns.
  2. Demonstrated knowledge in nutrition related to Diabetes Care and management.
  3. Demonstrated ability to operate and provide nutrition services via Ontario Telehealth Network (OTN) equipment.
  4. Demonstrated ability to problem solve and work efficiently under time constraints.
  5. Demonstrated excellent judgment and proven analytical skills.
  6. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  7. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  8. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  9. Demonstrated superior interpersonal and communication skills, both written and verbal.
  10. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  11. Demonstrated discretion and maturity when handling confidential information.
  12. Demonstrated commitment to the safety of co-workers and patients.

  9. Ability to travel between local sites and willingness to travel occasionally within Northern Ontario.
  10. Valid driver’s license with access to vehicle may be required.
