KEY FUNCTION:
Deliver nutrition therapy and provide services based on standards of quality and evidence-based practice to Outpatient Pediatric Services including, Pediatric Obesity, Hyperlipidemia, Feeding, Complex Care, Diabetes and Nutritional Counselling Clinics.
REPORTING:
Under the general direction of the Clinical Manager, Pediatric Outpatient Services.
DUTIES:
- Collect and interpret patient health information.
- Determine level of nutrition risk and prioritize caseload according to standards of care.
- Formulate a nutrition care plan with the support of the patient and family.
- Provide nutritional counseling and education on therapeutic diets related to the patient’s surgical condition and medical history.
- Work in collaboration with the food service department to adjust food items provided to the patient to ensure they are clinically indicated.
- Calculate nutrition need based on anthropometric data, previous and current medical history, blood work and other pertinent information.
- Design appropriate and safe enteral and parenteral nutrition care plans to meet the individual needs of the patient; monitor and evaluate the effectiveness of the nutrition interventions and adjust the care plan accordingly.
- Design appropriate and safe nutrition care plan to address failure to thrive diagnosis and eating disorders across all age ranges.
- Provide educational workshops to pediatric patients, their siblings and parents/guardians.
- Participate in onboarding, training and mentorship of new dietitian staff as required.
- Participate in the Dietetic Practicum Program through the Northern Ontario School of Medicine by preceptoring dietetic learners as required.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes as requested.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program or profession on various committees and in meetings as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a four (4) year Bachelor’s Degree in Nutrition and Dietetics, from an accredited university.
- Current Certificate of Registration in good standing with the College of Dietitians of Ontario is required.
- Successful completion of a Dietetic Internship Program is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of two (2) years’ experience working in a hospital setting or other similar health care environment.
- Minimum of two (2) years’ experience in pediatric nutrition care.
- Minimum of one (1) year experience working with pediatric diabetes.
- Experience in providing enteral and parenteral nutrition support is preferred.
- Experience working with children across all age ranges from pre-term to adolescents is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of pediatric obesity including evidence-based treatment approaches and familiarity with the Health at Every Size (HAES) framework.
- Proficiency in managing pediatric diabetes (i.e. carbohydrate counting, nutrition therapy, and current treatment modalities).
- Demonstrated strong knowledge of hyperlipidemia in children and evidence-based dietary interventions to support lipid management.
- Demonstrated knowledge of the nutrition care process (i.e. nutrition assessment, diagnosis, intervention and monitoring and evaluation).
- Demonstrated client-centered, individual counselling skills for for children, adolescents and caregivers.
- Demonstrated in-depth knowledge of pediatric, neonatal, enteral and parenteral nutrition.
- Demonstrated proficiency in calculating daily nutrition requirements (i.e. macro-and-micro-nutrients) and assessing adequacy of food intake for pediatric patients.
- Demonstrated knowledge of medical terminology including surgical terminology.
- Demonstrated knowledge of refeeding syndrome and strategies to manage the risk.
- Demonstrated knowledge of therapeutic diets (e.g. dysphagia diet).
- Demonstrated knowledge of eating disorders and appropriate treatment plans.
- Demonstrated ability to provide educational workshops and therapeutic group sessions.
- Demonstrated ability to collaborate with organizational and community partners to provide coordinated care.
- Demonstrated expertise in food literacy, with the ability to effectively teach nutrition skills and concepts.
- Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
