KEY FUNCTION:
Coordinate and liaise with Indigenous communities and service providers in implementing initiatives to improve the cancer journey and facilitate the outcomes of the North East (NE) Indigenous Cancer Plan of First Nation, Inuit, and Métis (FNIM) peoples in the NE region in alignment with HSN’s Strategic Plan and Indigenous Health priorities.

 

REPORTING:
Under the general direction of the Director, Indigenous Health.

 

DUTIES:
1. Identify priority health concerns and health equity gaps for Indigenous Peoples; focus on implementation of the Indigenous Cancer Strategy in the North East (NE) region.
2. Engage and partner with humility, valuing individual and community strengths and ideas to bring the best care, education and research solutions forward.
3. Seek community engagement and stakeholder participation to identify priorities.
4. Promote cultural safety in all engagement, educational and culturally relevant service provisions; promote positive care, work and learning experiences and foster the development of productive relationships with FNIM partners.
5. Coordinate project activities related to FNIM cancer screening, prevention, palliative care, and education.
6. Monitor project deliverables, compile and analyze data to support prioritized work plans.
7. Work collaboratively with FNIM and non-FNIM partners to support Indigenous cancer plan activities.
8. Support the delivery of culturally appropriate education and outreach to FNIM peoples in the NE region.
9. Act as a resource on FNIM issues relating to the NE Indigenous cancer plan for health care providers and community stakeholders.
10. Coordinate and chair meetings with affiliated working groups; work collaboratively to support NECC Navigator and Regional Indigenous Cancer Leads to advance the implementation of the NE Regional Indigenous cancer plan.
11. Support Indigenous engagement activities associated with HSN’s Strategic Plan including health and social service partners together with Indigenous Peoples for the shared purpose of improving health outcomes of Indigenous Peoples.
12. Plan and participate in Indigenous engagement events throughout the NE and on HSN’s Indigenous Health Advisory Council and/or Health Priority Sub-Committees.
13. Monitor approved annual budgets and manage fiscal accountability of the departmental operation in conjunction with Director and partnership agreement(s).
14. Educate and promote health, safety and wellness in the work place.
15. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
16. Participate in the accreditation process and work to ensure that the program and HSN achieve, maintain and continually improve upon their accredited status.
17. Teach, coach and lead staff in the development and monitoring of quality indicators and improvement initiatives.
18. Engage in and foster professional development of all staff and provide opportunities for learning within budgeted resources.
19. Ensure a safe environment for patients, staff and visitors; investigate, report, debrief and take or direct corrective action as required on incidents.
20. Collaborate with internal and external stakeholders/organizations/community groups to achieve program objectives, integrate and link services and foster partnerships across the continuum of service delivery.
21. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
22. Represent the department or program on various committees and in meetings as required.
23. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a three (3) year Bachelor’s Degree in a health-related field, from an accredited university.
2. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of three (3) years’ experience working in a health care or social service environment.
2. Minimum of two (2) years’ experience working with Indigenous communities, organizations or groups.
3. Experience working with FNIM language, culture, customs, and traditions is required.
4. Experience in writing and developing business documents (e.g. briefing notes, project reports, and presentations) is required.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of Ontario’s FNIM history, culture, health and socio-economic issues and needs.
2. Demonstrated ability to promote and model the Seven Grandfather Teachings.
3. Demonstrated knowledge of FNIM governance structures and protocols, organizations, communities, and geographic territories (e.g. NE region).
4. Demonstrated knowledge of oncology services and ethical issues as they relate to FNIM cancer patient populations.
5. Demonstrated excellent presentation, leadership and management experience.
6. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
7. Demonstrated ability to coach, advise and teach others using the principles of adult learning.
8. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
9. Demonstrated ability to independently identify issues, plan improvements, measure success and continue to develop Indigenous engagement approaches.
10. Demonstrated knowledge of relevant legislation and principle of management, hospital committee structure, conflict management and resolution, system needs analysis and organizational assessment.
11. Demonstrated ability to use tact and discretion in dealing with health care providers, community members and employees.
12. Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
13. Demonstrated superior interpersonal and communication skills, both written and verbal.
14. Demonstrated commitment to the safety of co-workers and patients.
