KEY FUNCTION

Work collaboratively with a multidisciplinary team and Physician Specialist to provide safe and effective assessment, treatment, and education for clients in rehabilitation to increase functional capacity, level of independence, and promote return-to-work.

 

REPORTING

Under the general direction of the Coordinator, Rehabilitation Business Development.

 

DUTIES

  1. Assist in the development of work hardening programs to increase client functional performance at home and work.
  2. Develop and implement recommendations to facilitate return-to-work and activities of normal living.
  3. Supervise therapeutic exercise sessions for outpatients of the Specialty Clinic and Forward To Function Group and offer vocational rehabilitation counselling.
  4. Design and supervise exercise sessions, develop exercise prescriptions based on the results of a graded exercise assessment, and participate in emergency responses.
  5. Provide initial and follow-up assessments, goal setting, and motivational support to clients and families regarding long-term behavioural modification.
  6. Communicate verbally or through written reports with other health care professionals regarding patient management for clinical problem-solving and program development.
  7. Refer clients to other members of the interdisciplinary team and participate in case conferences, team huddles, and team meetings.
  8. Design and deliver group education sessions both in a classroom setting and via videoconference.
  9. Liaise and work with other agencies and physicians’ offices to coordinate patient care when required.
  10. Act as a Kinesiology preceptor for post-secondary students.
  11. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  13. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  14. Educate and promote health, safety and wellness in the work place.
  15. Represent the department or program on various committees and in meetings as required. 
  16. Perform other duties as required. 

 

QUALIFICATIONS

EDUCATION AND TRAINING: 

  1. Minimum of a four (4) year Bachelor’s Degree in Kinesiology or Human Kinetics, from an accredited university.
  2. Current Certificate of Registration in good standing with the College of Kinesiologists of Ontario (COKO).
  3. Certificate in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED).
  4. Current Certificate of Registration with the Ontario Kinesiology Association (OKA) is preferred.
  5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of one (1) year full-time experience working in exercise interventions with patients within the last five (5) years.
  2. Minimum of one (1) year full-time experience working in adult education within the last five (5) years.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated ability to actively participate in exercise leadership, including resistance training and hydrotherapy in a pool setting.
  2. Demonstrated organizational and prioritization skills.
  3. Demonstrated excellent judgment and proven analytical skills.
  4. Demonstrated training, experience or utilization of lean methodology for process improvement.
  5. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  6. Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  7. Demonstrated superior interpersonal and communication skills, both written and verbal.
  8. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
  9. Demonstrated discretion and maturity when handling confidential information.
  10. Demonstrated commitment to the safety of co-workers and patients.
