KEY FUNCTION:
Work collaboratively with a multidisciplinary team and Physician Specialist to provide safe and effective assessment, treatment, and education for clients in rehabilitation to increase functional capacity, level of independence, and promote return-to-work.
REPORTING:
Under the general direction of the Coordinator, Rehabilitation Business Development.
DUTIES:
- Assist in the development of work hardening programs to increase client functional performance at home and work.
- Develop and implement recommendations to facilitate return-to-work and activities of normal living.
- Supervise therapeutic exercise sessions for outpatients of the Specialty Clinic and Forward To Function Group and offer vocational rehabilitation counselling.
- Design and supervise exercise sessions, develop exercise prescriptions based on the results of a graded exercise assessment, and participate in emergency responses.
- Provide initial and follow-up assessments, goal setting, and motivational support to clients and families regarding long-term behavioural modification.
- Communicate verbally or through written reports with other health care professionals regarding patient management for clinical problem-solving and program development.
- Refer clients to other members of the interdisciplinary team and participate in case conferences, team huddles, and team meetings.
- Design and deliver group education sessions both in a classroom setting and via videoconference.
- Liaise and work with other agencies and physicians’ offices to coordinate patient care when required.
- Act as a Kinesiology preceptor for post-secondary students.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a four (4) year Bachelor’s Degree in Kinesiology or Human Kinetics, from an accredited university.
- Current Certificate of Registration in good standing with the College of Kinesiologists of Ontario (COKO).
- Certificate in Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED).
- Current Certificate of Registration with the Ontario Kinesiology Association (OKA) is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of one (1) year full-time experience working in exercise interventions with patients within the last five (5) years.
- Minimum of one (1) year full-time experience working in adult education within the last five (5) years.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated ability to actively participate in exercise leadership, including resistance training and hydrotherapy in a pool setting.
- Demonstrated organizational and prioritization skills.
- Demonstrated excellent judgment and proven analytical skills.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
