Purpose: The Nurse Practitioner (NP) will function in an expanded role in an interdisciplinary practice model. The NP will work collaboratively with a physician(s) by managing, directing and providing comprehensive care for diabetes patients in the out-patient, in-patient programs, including consultations with regional partners as part of the Complex Diabetes Program. All duties and responsibilities of the NP will conform to professional practice standards, and hospital guidelines.
Standard Qualifications:
- A graduate from an approved Nurse Practitioner Program.
- Current certificate of competence in the extended class with the College of Nurses of Ontario.
Required Qualifications:
- Current certification in Basic Cardiac Life Support BCLS.
- Current certification in Life Saving Drugs.
- Current certification in ECG Level I and II.
- Current certification in Electrical Defibrillation.
Experience:
- Minimum of 3 years full-time equivalent experience (within the last 5 years) in medical nursing.
- Minimum of 2 years full-time equivalent experience (within the last 5 years) in adult education.
- Minimum of 3 years full-time equivalent experience (within the last 5 years) working with diabetes patients.
- Experience in developing, implementing and evaluating nursing practise tools in an ambulatory setting and primary care framework.
- Expertise in developing strategies to build capacity and improve chronic disease self-management outcomes.
- Demonstrated ability to work closely in an integrated team – minimum of 2 years full-time equivalent experience (within the last 5 years).
- Demonstrated leadership and critical thinking skills.
General Skills:
- Demonstrated ability for independent action, initiative, problem-solving, organization and prioritization.
- Demonstrated excellence in interpersonal and communication skills (written and oral).
- Demonstrated commitment to patient and family-centered care.
- Demonstrated ability to promote professional development.
- Ability to travel between local sites and/or valid driver’s license with access to vehicle may be required.
Preference Given to:
- Working knowledge of the Canadian Diabetes Association Clinical Practice Guidelines for the Management of Diabetes.
- Certification in Advanced Cardiac Life Support ACLS.
- Computer skills including Microsoft Office software.
- Working knowledge of videoconferencing media.
