KEY FUNCTION:
Practice, consult, educate and advocate in an interdisciplinary care team in implementation, evaluation and provision of high-quality, patient-centred orthopaedic care in collaboration with Medical Directors, other physicians and agencies. Support alignment of clinical best practice to achieve good clinical outcomes, Patient Related Outcome Measures (PROMs) and best practice length of stay.

 

REPORTING:
Under the general direction of the Clinical Manager, Orthopaedics/Neurosurgery and accountable to the Orthopaedic Surgeons.

 

DUTIES:
1. Provide daily rounding and communication with most responsible person (MRP) surgeons for orthopaedic patients, formulating a collaborative, interdisciplinary plan of care.

2. Participate in daily handover/bullet rounds.
3. Liaise with other services such as hospitalist program physicians, anesthesiology, internal medicine, general surgery, acute pain service, social work, physiotherapy, palliative care and geriatricians as required.
4. In consultation with MRP and patient care team members, facilitate consults to other services and facilitate care plans through patient care continuum including potential telephone correspondence.

5. Participate in family meetings as required.
6. Correspond by telephone with receiving hospital physicians to facilitate transfer back to home hospital and complete facility to facility medication reconciliation as required.
7. Ensure completion of outpatient physiotherapy referrals.

8. Ensure completion of discharge summaries, discharge prescriptions and completion of discharge medication reconciliation as required.
9. In consultation with MRP, during regular working hours, assess urgent/emergent patients including ordering and reviewing diagnostics, ordering medications and initiating appropriate consultation services.
10. For inpatients, assess, order and review diagnostic tests and imaging.

11. Communicate with community pharmacy regarding special program approvals for prescriptions.
12. Complete health care forms and referrals requiring physician/nurse practitioner level completion.
13. Consult, refer, collaborate and act as resource to members of the orthopaedic interprofessional team and community services.

14. Develop and maintain professional working relationships with all interprofessional team members and community agencies.
15. Assess population health trends and patterns and participate in designing services that promote healthy living.

16. Provide leadership in the development, implementation and evaluation of strategies to promote health and prevent illness and injury.
17. Attend Nurse Practitioner Professional Council Meetings, and as required, sit on and attend board, various committees and meetings as required.
18. Assist in the development and implementation of standards, guidelines, policies and procedures as well as medical directives for the orthopedic service.

19. Participate in pertinent evaluation and research studies related to patient’s healthcare, NP programs, practice and roles.
20. Determine and align improvement projects with HSN‘s Strategic Plan; monitor and adjust to achieve goal outcomes.
21. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.

22. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
23. Educate and promote health, safety and wellness in the work place.
24. Represent the department or program on various committees and in meetings as required.

25. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Graduate from an accredited Nurse Practitioner Program.
2. Current Extended Class Certificate of Registration in good standing with the College of Nurses of Ontario.
3. Current Basic Cardiac Life Support (BCLS) certificate is required.
4. Current certification in Automated External Defibrillator (AED) is required.
5. Master’s or Doctoral degree in nursing or related field is preferred.
6. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of three (3) years of experience as a Nurse Practitioner.

2. Experience in the development, implementation and evaluation of relevant advanced nursing practices.
3. Experience in the development of strategies to build capacity and improve health outcomes.
4. Experience in the development of clinical guidelines and protocols, promoting the use of research and evidence-based practice to guide patient-centered care.

5. Experience in the assessment, evaluation and initial management of surgical/orthopaedic patients.
6. Recent teaching experience of (adult/child) clinical education programs.

1. Demonstrated ability to integrate in-depth knowledge of advanced nursing practice and theory, health management, health promotion, disease/injury prevention and other relevant biomedical and psychosocial theories to provide comprehensive health services.

3. Demonstrated knowledge regarding the nursing process and its application (i.e. the assessment, planning, implementation and evaluation of surgical patients).
4. Demonstrated strong health assessment and physical examination skills.

6. Demonstrated ability to address patients questions and concerns with empathy;
7. Demonstrated ability to respect patient confidentiality and privacy,

8. Demonstrated superior interpersonal and communication skills, both written and verbal.
9. Demonstrated time management skills with the ability to manage workload and adapt quickly to a fast-paced, dynamic work environment.
10. Demonstrated ability to work effectively both independently and in a collaborative practice model as a member of an inter-professional team.

11. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
12. Demonstrated knowledge and understanding of the relevant acts, regulations, legislation and standards related to Occupational Health & Safety in the hospital setting.
13. Demonstrated ability to assist clients with concern and empathy, confidentiality and privacy, communicate in a courteous and respectful manner, and consider intended audience.

14. Demonstrated commitment to the safety of co-workers and patients.
15. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
