Purpose: Function in an expanded role in a collaborative interdisciplinary practice model with a primary focus on implementation, evaluation and provision of high-quality, patient-centred care. Support alignment of clinical best practice to achieve good clinical outcomes.
Standard Qualifications:
- Graduate from an accredited Nurse Practitioner Program.
- Current Certificate of Registration as RN (Extended Class) in good standing with the College of Nurses of Ontario.
Required Qualifications:
- Current Basic Cardiac Life Support (BCLS) certificate is required.
- Current certification in Automated External Defibrillator (AED) is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
Experience:
- Minimum of two (2) years’ of experience as a Primary Health Care Nurse Practitioner Practitioner
- Recent relevant experience in assessing medicine patients with complex medical needs is required.
- Experience in the development, implementation and evaluation of relevant advanced nursing practices is required.
- Experience in the development of strategies to build capacity and improve health outcomes is prefered
- Experience in the development of clinical guidelines and protocols, promoting the use of research and evidence-based practice to guide patient-centered care is prefered
General Skills:
- Work autonomously, concurrently or in consultation with other health care professionals to provide in-patient care and services related to General Internal Medicine (GIM) in collaboration with the clinical teaching unit (CTU); provide support within the GIM discharge clinic as required.
- Demonstrated ability to integrate in-depth knowledge of advanced nursing practice and theory, health management, health promotion, disease/injury prevention and other relevant biomedical and psychosocial theories to provide comprehensive health services.
- Provide direct comprehensive care to clients to manage acute and chronic illness with a focus on best practice approaches to senior’s friendly care.
- Provide leadership in the development, implementation and evaluation of strategies that aim to promote health, reduce length of stay and prevent readmission to the hospital.
- Demonstrated knowledge regarding the nursing process and its application, including the assessment, planning, implementation and evaluation of nursing care.
- Demonstrated leadership abilities with high levels of critical and logical thinking, analysis and/or reasoning to identify underlying principles, reasons or facts.
- Demonstrated high levels of initiative, resourcefulness, flexibility and adaptability.
- Demonstrated health assessment/clinical skills.
- Demonstrated commitment to patient and family-centred care.
- Demonstrated ability to work effectively both independently and in a collaborative practice model as a member of an inter-professional team.
- Demonstrated knowledge and understanding of the relevant acts, regulations, legislation and standards related to Occupational Health and Safety in the hospital setting.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated time management skills with the ability to manage workload and adapt quickly to a fast-paced, dynamic work environment.
- Demonstrated ability to assist clients with concern and empathy, respect their confidentiality and privacy, communicate in a courteous and respectful manner, and consider intended audience.
- Demonstrated commitment to the safety of co-workers and patients.
Preference Given to:
- Recent teaching experience of applicable (adult/child) clinical education programs is preferred.
