KEY FUNCTION

Provide wound care for patients living with Diabetes in the outpatient setting.  Provide specialized holistic assessment, prevention and management strategies and advocacy across the continuum of care for those living with chronic wounds. Coordinate outpatient wound services with community partners, provide treatment, education, and care to high-risk diabetes patients.

 

REPORTING

Under the general direction of the Coordinator, Diabetes Care, Bariatrics, Nutrition Counselling and Ophthalmology.

 

DUTIES

  1. Provide assigned patients with a high degree of individualized, effective, and ethical nursing care, utilizing advanced education, expert clinical skills and practice.
  2. Work in collaboration with a multidisciplinary team to assess patient health problems and needs, develop and implement nursing care plans, and educate patients on their illness and condition.
  3. Apply the Standards of Nursing Practice as outlined by the College of Nurses of Ontario.
  4. Provide all aspects of advanced wound care to high-risk diabetes patients.
  5. Provide footcare education to patients and diabetes care providers to enhance their knowledge and skills.
  6. Maintain standards of patient care as outlined for the unit.
  7. Maintain and adhere to HSN policies, procedures and standards of nursing.
  8. Support the Medical Professional in systematically assessing the health status of the individual by ordering, interpreting, and evaluating diagnostic tests.
  9. Develop plan of care with the help of other healthcare professionals to meet patient’s needs and communicate actions to maintain continuity among the nursing team.
  10. Ensure the safe and efficient implementation of nursing care plans.
  11. Monitor, record, and report to concerned personnel on patient care reflecting nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress.
  12. Systematically and continuously, evaluate the extent to which the individual’s health needs are being met and modify plan of care as indicated by patient’s response and condition.
  13. Provide patient and family with counselling, advice on health maintenance, and arrange for the continuation of required care after patient discharge; refer to community resources as required.
  14. Attend regular in-service sessions, conferences and seminars to keep abreast of new developments, trends and techniques in the nursing field and attend sessions related to specialties as required.
  15. Participate in long-term unit goals, quality control activities and process improvement initiatives as required.
  16. Liaise with other health care providers and community agencies to provide optimal patient care.
  17. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  18. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  19. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  20. Educate and promote health, safety and wellness in the work place.
  21. Represent the department or program on various committees and in meetings as required. 
  22. Perform other duties as required. 

 

QUALIFICATIONS

 

EDUCATION AND TRAINING: 

  1. Minimum of a graduate from an approved School of Nursing.
  2. Current Certificate of Registration in good standing with the College of Nurses of Ontario (CNO).
  3. Current Canadian Nurses Association (CNA) certification in Wound, Ostomy and Continence Certified (C)anada (WOCC(C)).
  4. Current certification in Basic Cardiac Life Support (BCLS).
  5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of two (2) years full time equivalent experience (within the last five (5) years) working in an outpatient setting with patients living with Diabetes .
  2. Minimum of two (2) years full time equivalent experience (within the last five (5) years) caring for patients with complex wounds.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated positive interpersonal relations and patient-teaching skills.
  2. Demonstrated commitment to patient and family-centered care.
  3. Demonstrated ability to effectively make decisions, deploy critical thinking and other skills required to organize/prioritize tasks.
  4. Demonstrated strong therapeutic communication skills and ability to communicate both verbally and in writing in a clear, concise manner, considering the intended audience.
  5. Demonstrated ability to work independently and interdependently.
  6. Demonstrated computer skills including Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  7. Demonstrated knowledge of the functional organization of the hospital and community agencies in providing care to the patient.
  8. Demonstrated ability to provide full-age spectrum care and liaise with other programs and care providers.
  9. Proven knowledge of different patient care delivery trends as applied to the nursing unit.
  10. Demonstrated commitment to the safety of co-workers and patients.
