Health Sciences North (HSN) is seeking an innovative individual for the position of an Occupational Therapist to provide high intensity rehabilitation programming, with a focus on neurological rehabilitation for outpatients, post-discharge from hospital. They will have the opportunity to use a holistic, client, and family-centred approach to optimize safety, functional ability, and well-being to enable participation in activities of daily living within the areas of self-care, productivity and leisure, with an emphasis on the physical, cognitive, affective, and spiritual components of clients’ functional performance and the physical, institutional, social, and cultural aspects of the environment.
The ideal candidate will have a minimum of a current Certificate of Registration in good standing with the College of Occupational Therapist. However, candidates expected to obtain registration in the next few months and recent graduates are strongly encouraged to apply. Training as an Assistive Devices Program (ADP) Authorizer and completion of the CorHealth’s Inter-professional Stroke Core Competency Framework or willingness to complete within one month of employment is required.

REHABILITATION PROFESSIONALS INCENTIVE GRANT
The incentive grant program offers taxable incentive grants of up to $5,000 per eligible year, to a maximum of $15,000 paid over a three-year period to occupational therapists who establish their practice in Northern Ontario and accept full-time positions which are fully funded by the Ministry of Health and Long-Term Care (MOHLTC) and which are approved by the Northern Health Programs (NHP). In order to be eligible for this incentive grant, the NHP must approve the application before the applicant establishes his/her practice. More information regarding this grant can be found here.

KEY FUNCTION:

Emphasize the physical, cognitive, affective, and spiritual components of clients’ functional performance and the physical, institutional, social, and cultural aspects of the environment.
REPORTING:
Under the general direction of Coordinator, Outpatient Neurological Rehabilitation and Outpatient Brain Injury Rehabilitation Services.
DUTIES:
- Support implementation and adoption of CorHealth Ontario’s Model of Care for Community Stroke Rehabilitation.
- Participate in the intake and/or transitonal care processes for referred patients from hospital.
- Administer and perform standardized and non-standardized assessments.
- Analyze and interpret assessment findings.
- Generate, implement, evaluate, and modify treatment plans.
- Conduct home and/or community visits for assessments and/or treatments.
- Participate in or support a virtual model of therapeutic care as required.
- Recommend and educate compensatory and remedial strategies, activity modification, adaptive equipment, and assitive devices to promote safety and maximize functional independence.
- Dispense, fabricate, or modify equipment, including wheelchairs,and splints.
- Implement strategies to address client safety issues.
- Educate client and family in coping strategies, decision-making, and utilization of personal and community resources, and make referrals to community resources as required.
- Collaborate with clients, families, interprofessional team members, and community partners throughout the therapy process and advocate to ensure client needs are met.
- Participate in the service’s discharge planning process.
- Document assessment findings, analysis, treatment plans, client goals, client’s ongoing progress, effectiveness of treatment plan, and government/insurance/legal forms according to the College of Occupational Therapists of Ontario (COTO) standards and HSN policy.
- Provide coverage support in the Outpatient Rehabilitation service to meet patient care demands as required.
- Assign interventions and provide guidance to support personnel in accordance with the COTO standards.
- Research and incorporate best evidence into clinical practice, and conduct or collaborate in research.
- Educate and provide internships to student Occupational Therapists (OT)/Occupational Therapist Assistants/Physical Therapist Assistants/Rehab Assistants, other members of an interprofessional health care team and their students, external health care professionals, and school/community staff/agencies.
- Ensure the privacy and confidentiality of personal health information is maintained.
- Maintain the safety of the client, OT, and other staff.
- Recommend, manage, and maintain inventory, and order essential supplies and equipment as required.
- Maintain required data and relevant statistical information.
- Follow service-specific, Occupational Health and Safety (OH&S) and Infection Prevention and Control practices, including monthly workplace inspections.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety and Occupational Therapy practice legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Current Certificate of Registration in good standing with the COTO.
- Training as an Assistive Devices Program (ADP) Authorizer is required.
- Completion of the CorHealth’s Interprofessional Stroke Core Competency Framework or willingness to complete within one month of employment is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate.
EXPERIENCE:
- Experience working with people who have had a stroke, spinal cord injury, other neurological conditions or muskuloskeletal disorders in a rehabilitation setting is required.
- Experience working with an evidence-based approach and within an interdisciplinary team to service delivery is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of Occupational Therapy theory, other theoretical frameworks relevant to the area of practice, and the Canadian Practice Process Framework (CPPF).
- Demonstrated knowledge and application of client and family-centred models of care.
- Demonstrated knowledge of COTO standards of practice and guidelines.
- Demonstrated knowledge of current health care and privacy legislation.
- Demonstrated knowledge of evidence-based clinical practice and best practice.
- Demonstrated knowledge of outcome measures and research process/methodology.
- Demonstrated knowledge of community services and available resources.
- Demonstrated ability to exercise professional judgment, clinical reasoning, and critical thinking with the ability to apply ethical frameworks and choose the optimal assessment and treatment intervention.
- Demonstrated ability to direct, supervise, and mentor support personnel, staff, and students.
- Demonstrated ability to effectively problem-solve, set priorities, and manage conflict.
- Demonstrated knowledge of Occupational Therapy techniques relevant to the area of practice.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
