KEY FUNCTION:
Provide assertive community outreach and actively participate on a multidisciplinary team to provide direct client services for people who are living with serious, complex mental illness, and who require long-term support.
REPORTING:
Under the general direction of the Clinical Manager, Psychosocial Rehabilitation.
DUTIES:
1. Provide evidence-based treatment for individuals in the Psychosocial Rehabilitation programs.
2. Complete intake assessments.
3. Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.
4. Document assessments, treatment plans, progress notes, and outcomes in a timely manner.
5. Assist in and act as a resource for client and family education.
6. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
7. Provide guidance, training and orientation for new staff and students.
8. Work collaboratively with community providers to provide wrap around care for complex clients.
9. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
10. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
11. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
12. Educate and promote health, safety and wellness in the work place.
13. Represent the department or program on various committees and in meetings as required.
14. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a one (1) year Diploma in a human services field, from an accredited college.
2. Certification in Peer Support Training from a recognized training provider is preferred.
3. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of three (3) years’ recent experience in community-based, outpatient programs.
2. Lived experience of mental health challenges, which may include one’s own direct experiences or supporting a family member living with these challenges, is required.
3. Experience working with adults with severe and persistent mental illness is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge and understanding of principles of recovery as it applies to mental health and addictions treatment.
2. Demonstrated excellent judgment and proven analytical skills.
3. Demonstrate strong client and family service orientation.
4. Demonstrated knowledge of culturally safe practices.
5. Proven ability and experience to manage rapidly changing mental health states and psychiatric emergencies
6. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
7. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
8. Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
9. Demonstrated superior interpersonal and communication skills, both written and verbal.
10. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
11. Demonstrated discretion and maturity when handling confidential information.
12. Demonstrated commitment to the safety of co-workers and patients.
PERSONAL SUITABILITY:

2. Ability to use tact and discretion in dealing with health care providers and patients.
