KEY FUNCTION:
Provide direct and indirect care to patients, supporting the functions of all team members. Enhance and expand the teams’ ability to provide safe, efficient, effective, and comprehensive quality care.
REPORTING:
Under the general direction of the Clinical Manager, Reactivation Care.
DUTIES:
1. Implement therapeutic, geriatric-focused care of assigned patients with the goal of identifying restorative potential.
2. Ensure appropriate scheduling and admission of each client.
3. Provide initial and ongoing assessment of patient and family needs.
4. Coordinate administrative issues and reception desk including patient call lights and involve RN as required.
5. Assist patients in their personal care, physical exercise, personal hygiene and all activities of daily living (i.e., bathing, dressing, feeding, and toileting).
6. Interact with patients, families and visitors providing information and support.
7. Collaborate with other health care providers in delivering and reporting daily patient care needs by maintaining records, observing or monitoring and recording patients' status.
8. Protect and promote the welfare, dignity, and individual rights of patients at all times.
9. Perform minor maintenance tasks such as assist with the set-up, operation and maintenance of equipment.
10. Assist with collection of patient data (i.e. height, weight and measurement of intake and output).
11. Make occupied and unoccupied beds and ensure dirty linen is placed in the dirty utility room.
12. Report any changes in patient condition immediately to the RN, RPN.
13. Operate mechanical lifts and utilize assistive devices as required.
14. Provide each patient with meaningful companionship, observation, safety and conservation.
15. Maintain a clean, safe environment for residents utilizing infection control principles and identify, assess and communicate safety concerns to supervisor/manager or delegate.
16. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
17. Participate in senior friendly care learning series and other geriatric courses as required.
18. Participate in hospital training as required.
19. Assist with improvement initiatives as required.
20. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a Personal Support Worker Certificate from an accredited college or equivalent.
2. Current Basic Cardiac Life Support (BCLS) and Automated External Defibrillator (AED) certificate is required.
3. Current certification in Non-Violent Crisis Intervention (NVCI) and Gentle Persuasion Approaches to Dementia Care (GPA) is preferred.
4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of six (6) months’ experience working as a Personal Support Worker in a hospital setting.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated ability to effectively interact and communicate with patients, families, and healthcare workers.
2. Demonstrated ability to deal effectively and co-operatively with all levels of staff, patients, physicians, volunteers, and the public.
3. Demonstrated ability to work effectively and efficiently in a fast-paced environment.
4. Demonstrated working knowledge of equipment used in the delivery of patient care.
5. Demonstrated knowledge and understanding of the Ontario Hospital Association (OHA)/Ontario Medical Association’s (OMA) Communicable Disease Surveillance Protocols.
6. Demonstrated discretion and maturity when handling confidential information.
7. Demonstrated computer skills with intermediate proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and applicable patient information systems.
8. Demonstrated interpersonal and communication skills.
9. Ability to prioritize tasks.
10. Demonstrated compliance with HSN’s privacy policy.
11. Demonstrated commitment to the safety of co-workers and patients.
