Health Sciences North (HSN) is seeking an innovative individual for the position of Physiotherapist in Complex Diabetes department to provide specialized care to the complex diabetes patient population, to promote optimal health and to maintain or restore optimum function and independence. The successful incumbent will assess, treat, and provide education in an outpatient day hospital setting and offer various effective non-pharmacological approaches for pain relief.

The ideal candidate will have a minimum of a Current Certificate of Registration in good standing with the College of Physiotherapist of Ontario. However, candidates expected to obtain registration in the next few months and recent graduates are strongly encouraged to apply. We are looking for someone with a minimum of three (3) years’ recent related clinical experience. In addition, the successful candidate will have a minimum of two (2) years’ experience working in an in/outpatient department and working experience with high risk diabetes patients, within a health care environment.

The Diabetes program is an interdisciplinary team which promotes and provides access to timely quality diabetes education and care for people with Diabetes in Sudbury and Northern Eastern Ontario. At the Diabetes Program the person who has diabetes is recognized as the most important member of the diabetes care team.  The Diabetes program supports people with Diabetes as they self manage this chronic disease

REHABILITATION PROFESSIONALS INCENTIVE GRANT

 

The incentive grant program offers taxable incentive grants of up to $5,000 per eligible year, to a maximum of $15,000 paid over a three-year period to audiologists, chiropodists, occupational therapists, physiotherapists and speech language pathologists who establish their practice in Northern Ontario and accept full-time positions which are fully funded by the Ministry of Health and Long-Term Care (MOHLTC) and which are approved by the Northern Health Programs (NHP). KEY FUNCTION

Provide specialized care to the complex diabetes patient population to promote optimal health and to maintain or restore optimum function and independence. Assess, treat, and provide education in an outpatient day hospital setting. Offer various effective non-pharmacological approaches for pain relief, including Transcutaneous Electrical Nerve Stimulation (TENS).

 

REPORTING 

Under the general direction of the Coordinator, Diabetes, Bariatrics, Nutrition Counselling and Ophthalmology.

 

DUTIES:

  1. Perform detailed, comprehensive medical history and physical assessment of patients to establish a clinical impression for physiotherapy intervention when required.
  2. Initiate and execute all physiotherapy treatments related to the clinical impression to deliver patient care within various clinical areas, and perform on-going evaluations to determine when discharge from service is appropriate.
  3. Provide supervision and direction to Physiotherapy Assistants (PTA) and assigned support personnel as required in accordance with the standards defined by the College of Physiotherapists of Ontario (CPO).
  4. Document assessment in the patient’s medical chart/Patient Care System (PCS) documentation, clinical impression, and all related physiotherapy intervention and patient outcomes; maintain an accepted standard of documentation for quality assurance as outlined by HSN policies and the CPO.
  5. Communicate verbally or through written reports with other health care professionals regarding patient management for clinical problem-solving and program development; may be required to participate in Centre for Complex Diabetes Care (CCDC) patients rounds.
  6. Act as a professional resource for internal health care providers and community partners.
  7. Provide theoretical and clinical education/supervision of physiotherapy students and undergraduate students.
  8. Develop and implement self-learning objectives, participate in related postgraduate educational opportunities to improve clinical expertise, and integrate new knowledge to enhance patient care.
  9. Participate on patient care teams and other committees, including Professional Council meetings, regarding the development of policies, procedures, forms, and audits in collaboration with the team and supervisor.
  10. Complete home visits to identify the need for further physiotherapy services in the community (e.g. Home and Community Care) as required.
  11. Act as the case manager for patients in the CCDC as required.
  12. Educate and promote health, safety and wellness in the work place.
  13. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  14. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  15. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  16. Represent the department or program on various committees and in meetings as required. 
  17. Perform other duties as required. 

 

QUALIFICATIONS

 

EDUCATION AND TRAINING: 

  1. Current Certificate of Registration in good standing with the CPO.
  2. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE: 

  1. Minimum of three (3) years’ recent related clinical experience.
  2. Minimum of two (2) years’ experience working in an in/outpatient department within a health care environment.
  3. Minimum of two (2) years’ experience working with high risk diabetes patients.

 

KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated knowledge of diabetes and its associated co-morbidities.
  2. Demonstrated ability to direct, supervise, and mentor students and support personnel.
  3. Demonstrated current knowledge of trends in general health care and community resources.
  4. Demonstrated knowledge of Medical Terminology.
  5. Demonstrated excellent judgment and proven analytical skills.
  6. Demonstrated training, experience or utilization of lean methodology for process improvement.
  7. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  8. Demonstrated excellent computer skills with proficiency in Microsoft Office software, (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  9. Demonstrated superior interpersonal and communication skills, both written and verbal.
  10. Strong organizational skills with the ability to prioritize workload.
  11. Demonstrated discretion and maturity when handling confidential information.
  12. Demonstrated commitment to the safety of co-workers and patients.

 

PERSONAL SUITABILITY:
