KEY FUNCTION:
Facilitate and promote access to Low Back Care throughout the North East region. Through collaboration with local and regional spine surgeons, advanced practice provides, and primary care providers, conduct a comprehensive assessment and create a care plan for patients with low back disorders.
REPORTING:
Under the general direction of the Clinical Manager, Ambulatory Care Services, Medical Day Care, and MSK Assessment Centre.
DUTIES:
1. Act as a liaison between the clinic Surgeon Sponsor, Advanced Practice Providers (APP), Central Intake, PCPs, patients, LB RAC provincial implementation and operations team and other key stakeholders.
2. Act as a resource for the region’s APP - Provide clinical guidance and training, oversee, and supervise clinical and professional practice
3. Identify and order diagnostic laboratory tests and therapeutic procedures under authorization of medical directive
4. Work in an independent and interdependent relationship with members of the medical staff to determine health care plans.
5. Provide patient with a management/treatment plan including patient education on self-management, optimization of patient’s health, function and knowledge prior to surgery; initiate referrals to other members of the health care team based on assessment and referral guidelines.
6. Commuicate to referring physicians regarding patient outcomes.
7. Serveas the primary LB RAC contact for patients, HSN employees, and the community.
8. Participate in ongoing development of the program to meet needs of patients, specialists, and the community.
9. Ensure all prescribed clinical practice standards and guidelines are followed.
10. Participate in Inter-Professional Spine Assessment and Education Clinics (ISAEC) sponsored regional and central education and training events.
11. Develop and/or participate in practice-based research.
12. Participate in the recruitment, hiring, training and management of advanced practice providers throughout the North East region.
13. Demonstrate a strong understanding of the role of exercise prescription, self-management, and adjunct care
14. Develop plan of care in collaboration with the patient/ family; surgeon and primary care provider
15. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
16. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
17. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
18. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines and of the professional physiotherapy practice.
19. Educate and promote health, safety and wellness in the work place.
20. Represent the department or program on various committees and in meetings as required.
21. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Current Certificate of Registration with the college of Physiotherapists of Ontario (CPO) is required.
2. Completion of 12 weeks of practical and theoretical training in LBP topics, sponsored by the LB RAC, is required.
3. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of four (4) years’ clinical experience, including experience in the musculoskeletal – spine area.
2. Experience in an advanced practice role in an integrated primary care environment is preferred.
KNOWLEDGE / SKILLS / ABILITIES:
1. Demonstrated excellent judgment and proven analytical skills.
2. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
3. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
4. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems, as well as videoconferencing platforms for virtual care.
5. Demonstrated superior interpersonal and communication skills, both written and verbal.
6. Demonstrated strong organizational skills with the ability to prioritize workload.
7. Demonstrated discretion and maturity when handling confidential information.
8. Demonstrated commitment to the safety of co-workers and patients.
