KEY FUNCTION:
Establish and maintain a safe and therapeutic environment for patients in acute care and the inter-professional team.
REPORTING:
Under the general direction of the Coordinator, System Transformation and Alternate Health Care Facility.
DUTIES:
- Assist the clinical team in development and maintenance of a therapeutic environment.
- Implement and guide patients through recommended treatment and activity programs.
- Instruct patient in the use of equipment and adaptive devices.
- Assist with patient referrals and scheduling in accordance with unit practices.
- Maintain an inventory of equipment, devices and materials.
- Document patient intervention as per unit requirements.
- Report patient behaviour, progress, and concerns to assigned therapist.
- Work collaboratively with the clinical team to meet departmental/patient needs.
- Provide care during transition from acute care.
- Maintain relevant statistical information.
- Assist clinical team in the development and maintenance of a therapeutic environment.
- Evaluate patient care and satisfaction; formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a two (2) year Physiotherapy Assistant (PTA) Diploma, Occupational Therapist Assistant (OTA) Diploma, or Rehabilitation Assistant Diploma from an accredited college.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of two (2) years’ experience working with older adults in a specialized geriatric and/or rehabilitative setting.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of community support services, home and community care, rehabilitative services, adult day programs and other social services.
- Demonstrated ability to assist in development and maintenance of a therapeutic environment.
- Demonstrated ability to instruct and guide patients through recommended treatment and activity programs, both individually and groups.
- Demonstrated good judgment, organizational, and problem-solving skills.
- Demonstrated knowledge of available mobility equipment and its use.
- Demonstrated ability to safely handle and maintain equipment.
- Demonstrated ability to safely manage patients with medical equipment (e.g. invasive lines, oxygen tubing, telemetry/cardiac monitor, and chest tubes).
- Demonstrated knowledge of patient monitoring alarms.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software, e.g. Word, Excel, Power Point and Outlook and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
PERSONAL SUITABILITY:
- Successful Vulnerable Sector Check (VSC) is required.
- Demonstrated initiative with minimal supervision.
- Demonstrated commitment to ongoing professional development.
