KEY FUNCTION:
Assist team members in supporting and delivering therapeutic care to outpatients post-discharge from hospital. Assist in establishing and maintaining a safe and therapeutic environment through engagement and interaction with the patients.
REPORTING:
Under the general direction of Coordinator, Outpatient Neurological Rehabilitation and Outpatient Brain Injury Rehabilitation Services.
DUTIES:
- Assist the clinical team in the development and maintenance of a therapeutic environment.
- Work collaboratively with the clinical team to best meet the service or patient’s needs.
- Implement and guide outpatients through recommended treatment and activity programs both individually and in groups as directed by the therapist (e.g. receive and prepare the patients/clients for treatment, assist with treatment modalities and with person’s activities of daily living, provide support or assistance for walking, transfers, communication skills, etc.).
- Assist in providing assessments and/or treatments in individualized or group settings.
- Instruct the patient in the use of equipment, and adaptive devices as directed by the therapist.
- Assist with referrals and/or patient scheduling in accordance with the service’s mandate, under the direction of the therapist.
- Communicate regularly with the professional team about the client’s experience or relevant observations made during engagement with client.
- Maintain a safe environment by caring for equipment breakdowns, remove hazards, complete maintenance requisitions and notify therapist.
- Document patient intervention in the health record as per service requirements.
- Report observation of patient’s behaviour, progress and concerns to the assigning therapist.
- Maintain relevant data or statistical information.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a two (2) year Diploma in a related health care field, from an accredited college.
- Current Certificate as an Occupational Therapist Assistant (OTA), Physiotherapist Assistant (PTA), or a Rehabilitation Assistant is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of two (2) years’ experience working in a health care environment.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of recovery and patient-centered approaches.
- Demonstrated ability to assist in the development and maintenance of a therapeutic environment.
- Demonstrated ability to instruct and guide patients through recommended treatment and activity programs both individually and in groups.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
PERSONAL SUITABILITY:
- Successful Vulnerable Sector Check is required.
- Ability to use tact and discretion in dealing with health care providers and patients.
- Demonstrated ability to work effectively as a member of an interdisciplinary team.

- Ability to meet the physical and sensory demands of the job.
- Valid driver’s license with access to reliable vehicle is required.
- Ability to travel between local sites and regional sites (e.g. Patient Homes, Community Locations).
