KEY FUNCTION:
Assess patient’s recreation and leisure needs, develop treatment plans and programs, provide therapeutic programs and interventions to meet these needs, and evaluate the client’s progress.
REPORTING:
Under the general direction of the Clinical Manager, Integrated Chronic Pain, Outpatient Brain Injury Rehabilitation Services (OBIRS), Chronic Disease Self-Management and HAVEN/Hemophilia.
DUTIES:
- Develop a treatment plan to meet the needs of the patient based on needs assessment, patient interests, and objectives of therapy.
- Prepare and deliver education to people living with a chronic condition.
- Observe, analyze, and record patients' participation, reactions, and progress during treatment sessions; modify treatment programs as required.
- Encourage clients with special needs and circumstances to acquire new skills and get involved in health-promoting leisure activities.
- Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
- Minimum of a two (2) year Diploma in Therapeutic Recreation, from an accredited college.
- Current Certificate of Registration with the Therapeutic Recreation of Ontario (TRO) is preferred.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
- Minimum of two (2) years’ experience working in a related clinical department within a health care environment.
KNOWLEDGE/SKILLS/ABILITIES:
- Demonstrated knowledge of Community Agencies, Ministry of Health and Long Term Care (MoHLTC), and other health and social services and rehabilitation resources.
- Demonstrated knowledge of Medical Terminology.
- Demonstrated excellent judgment and proven analytical skills.
- Demonstrated ability to mentor therapy assistants.
- Demonstrated ability to provide education to people living with a chronic condition.
- Demonstrated knowledge and understanding of the Ontario Hospital Association (OHA)/Ontario Medical Association’s (OMA) Communicable Disease Surveillance Protocols.
- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
- Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
- Demonstrated superior interpersonal and communication skills, both written and verbal.
- Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
- Demonstrated discretion and maturity when handling confidential information.
- Demonstrated commitment to the safety of co-workers and patients.
PERSONAL SUITABILITY:
- Successful Vulnerable Sector Check is required.
- Ability to use tact and discretion in dealing with health care providers and patients.
- Demonstrated ability to work effectively as a member of an interdisciplinary team.
- Demonstrated ability to perform with minimal supervision; to prioritize duties.
- Demonstrated commitment to ongoing professional development.
- Demonstrated professionalism in dealing with confidential and sensitive issues.
- Demonstrated positive work record and good attendance record.
- Ability to meet the physical and sensory demands of the job.
- Ability to travel between local sites.
