REVISE AND REPOST 

KEY FUNCTION:
Deliver comprehensive, patient-centred oncology nursing care by applying advanced knowledge, clinical expertise, and ethical practice to support patients throughout their cancer journey.

 

REPORTING:
Under the general direction of the Manager, Systemic Treatment.

 

DUTIES:
1. Practice in accordance with the College of Nurses of Ontario Standards of Nursing Practice.
2. Maintain and adhere to HSN policies, procedures and nursing standards.
3. Collaborate with medical professionals to assess patient health status (e.g., order, interpret, and evaluate diagnostic tests).
4. Develop, implement, and coordinate individualized plans of care in collaboration with the interprofessional team to ensure continuity of care.
5. Ensure the safe and efficient implementation of care plans.
6. Continuously evaluate patient outcomes and revise care plans based on patient condition and response to treatment.
7. Monitor, document, and communicate patient status, responses to care, progress, and changing needs.
8. Provide patient and family education, counselling, and health maintenance guidance; coordinate discharge planning and referrals to community resources as required.
9. Participate in ongoing professional development, including in-services, conferences, seminars, and specialty-specific education.
10. Participate in studies and projects as required.
11. Support the development of nursing philosophy, objectives, and improvement initiatives in collaboration with management and aligned with HSN’s Strategic Plan.
12. Provide patient navigation across multiple cancer pathways, guiding individuals from referral through diagnosis and into appropriate cancer services.
13. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
14. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
15. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
16. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
17. Educate and promote health, safety, and wellness in the workplace.
18. Represent the department or program on various committees and in meetings as required.
19. Perform other duties as required.

 

QUALIFICATIONS
EDUCATION AND TRAINING:
1. Graduate from an approved School of Nursing.
2. Current Certificate of Registration in good standing with the College of Nurses of Ontario (CNO).
3. Current Certification in Basic Cardiac Life Support (BCLS), Automated External Defibrillator (AED), and Vascular Access Devised (VAD).
4. Successful completion of either the de Souza Provincial Standardized Chemotherapy and Biotherapy Course or Oncology Nursing Society/Oncology Nursing Certification Corporation (ONS/ONCC) Chemotherapy Immunotherapy Certificate Course, or must
be registered for the next available course offering prior to their start date, and renewal every two (2) years.

5. Successful completion of the Canadian Nurses Association (CNA) certification in Oncology Nursing or commitment to obtain within five (5) years.
6. Department-specific certification, training, and education may be required.
7. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:
1. Minimum of two (2) years’ full-time equivalent experience, within the last five (5) years, in Oncology Outpatient Clinics and/or Systemic Treatment delivery.
2. Minimum of two to three (2-3) years’ experience, within the last five (5) years, in an acute clinical care setting.

 

KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated positive interpersonal relations and patient-teaching skills.
2. Demonstrated commitment to patient and family-centered care.
3. Demonstrated ability to effectively make decisions, deploy critical thinking and other skills required to organize/prioritize tasks.
4. Demonstrated strong therapeutic communication skills and ability to communicate both verbally and in writing in a clear, concise manner, considering the intended audience.
5. Demonstrated ability to work independently as well as interdependently.
6. Demonstrated computer skills including Microsoft Office software and Meditech Client Service Order Entry.
7. Demonstrated knowledge of the functional organization of the hospital and community agencies in providing care to the patient.
8. Demonstrated ability to provide full-age spectrum care and liaise with other programs and care providers.
9. Proven knowledge of different patient care delivery trends as applied to the nursing department.
10. Demonstrated commitment to the safety of co-workers and patients.
