KEY FUNCTION:

Provide organizational support and patient care services in the Outpatient Rehabilitation Programs.

 

REPORTING:

Under the general direction of the Manager, Outpatient Rehabilitation Programs and under the supervision of a registered Physiotherapist and/or Occupational Therapist.

 

DUTIES:

  1. Assist the clinical team in the development and maintenance of a therapeutic environment; work collaboratively with the clinical team to best meet the service or patient’s needs.
  2. Implement and guide outpatients through recommended treatment and activity programs both individually and in groups as directed by the therapist (e.g. receive and prepare the patients/clients for treatment, assist with treatment modalities and with person’s activities of daily living, provide support or assistance for walking, transfers, etc.).
  3. Instruct the patient in the use of equipment, and adaptive devices as directed by the therapist.
  4. Assist with referrals and/or patient scheduling in accordance with the service’s mandate, under the direction of the therapist.
  5. Communicate regularly with the professional team about the client’s experience or relevant observations made during engagement with client.
  6. Report observation of patient’s behaviour, progress and concerns to the assigning therapist.
  7. Document patient intervention in the health record as per service requirements.
  8. Maintain a safe environment by caring for equipment breakdowns, remove hazards, complete maintenance requisitions and notify therapist.
  9. Maintain relevant data or statistical information.
  10. Ensure cleanliness and tidiness of equipment, environment, supply and storage areas, linen, and maintain inventory of equipment and treatment supplies.
  11. Follow service-specific and Occupational Health and Safety (OH&S) practices, including monthly workplace inspections.
  12. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  13. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  14. Educate and promote health, safety and wellness in the work place.
  15. Represent the department or program on various committees and in meetings as required. 
  16. Perform other duties as required. 

 

QUALIFICATIONS

EDUCATION AND TRAINING:

  1. Minimum of a two (2) year Diploma in a related health care field preferably from a Occupational Therapist Assistant (OTA), Physiotherapist Assistant (PTA), or a Rehabilitation Assistant program, from an accredited college.
  2. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

EXPERIENCE:

  1. Minimum of two (2) years’ experience working in a health care environment.

 

KNOWLEDGE/SKILLS/ABILITIES:

  1. Demonstrated ability to instruct and guide patients through recommended treatment programs both individually and in groups.
  2. Demonstrated ability to safely handle and maintain equipment, and fabricate devices (e.g. sewing).
  3. Demonstrated ability to assist patients in performance of therapeutic activities, work hardening, and scar management.
  4. Demonstrated good judgment and problem-solving skills.
  5. Demonstrated excellent presentation skills.
  6. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
  7. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  8. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  9. Demonstrated superior interpersonal and communication skills, both written and verbal.
  10. Demonstrated strong organizational skills with the ability to prioritize workload.
  11. Demonstrated discretion and maturity when handling confidential information.
  12. Demonstrated commitment to the safety of co-workers and patients.
