Implement the Senior Friendly Care Framework and the geriatric rehabilitative care capacity plan.  Teach and mentor front line clinicians in geriatric approach to care, including early identification of restorative potential and direct access priority processes.



Under the general direction of the Administrative Director, North East Specialized Geriatric Centre.



  1. Implement a sub-region approach to senior friendly care, geriatric rehabilitative care capacity planning, early identification of restorative potential and direct access priority processes.
  2. Implement the Senior Friendly Care Framework and develop geriatric clinical capacity across the local continuum according to best practices.
  3. Lead, facilitate and participate in complex seniors’ care planning meetings and committees across the Cochrane region (i.e. Rehab Capacity-Planning committee, Acute Care for Elders (ACE) strategy planning meetings, Senior Friendly Hospital Steering Committee, etc).
  4. Work collaboratively with health care providers and community support services (i.e. primary care, North East Local Health Integration Network (NELHIN) Home and Community Care, Alzheimer’s Society, Community Paramedicine, etc).
  5. Assess gaps and learning needs in geriatric care and develop and implement a plan to address geriatric knowledge and capacity.
  6. Coach the geriatric approach to patient care including patient engagement and transition planning. 
  7. Track quality indicators related to the sub region deliverables, monitor and report activities through the North East Specialized Geriatric Centre (NESGC) local and regional reporting structure and support statistical reporting of clinical activity as required.
  8. Facilitate and monitor the use of effective clinical tools, referral and communication processes.
  9. Administer and teach Comprehensive Geriatric Assessment and Treatment guided by the Interprofessional Comprehensive Geriatric Assessment (CGA) Competency Framework (e.g. Geriatric Medicine, Rehabilitation and Inpatient Consultation).
  10. Demonstrate the process of analyzing and synthesizing findings from the CGA to identify client’s priority needs and relevant treatment options designed to optimize independence.
  11. Engage in professional development and participate in opportunities for learning within budgeted resources.
  12. Educate and promote health, safety and wellness in the work place.
  13. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  14. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  15. Represent the department or program on various committees and in meetings as required. 
  16. Perform other duties as required. 




  1. Minimum of a four (4) year Bachelor’s Degree in a health related field, from an accredited university.
  2. Master’s degree in a health-related field is preferred.
  3. Current Certificate of Registration in good standing as a Health Professional with an Ontario Professional Association or regulatory college.
  4. Advanced training in specialized geriatrics is required.
  5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.



  1. Minimum of five (5) years’ experience working in specialized geriatrics across the care continuum.
  2. Experience in process improvement, change management and project planning is required.



  1. Demonstrated ability to work with geriatric patients with significant functional deficits and communication challenges (i.e. cognitive impairment, sensory impairment, behavioral problems or ethno-cultural pluralities).
  2. Demonstrated knowledge of clinical, socio-behavioural, and fundamental biomedical sciences relevant to geriatric syndromes and geriatric practice. 
  3. Demonstrated knowledge in patient and family-centered approaches in geriatric care planning and treatment.
  4. Demonstrated capacity building and facilitation skills.
  5. Demonstrated superior knowledge and understanding of managing key stakeholders, both within and outside the hospital setting.
  6. Demonstrated superior analytical, problem-solving, negotiation and conflict resolution skills.
  7. Demonstrated knowledge of the local service system.
  8. Demonstrated training, experience or utilization of lean methodology for process improvement.
  9. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  10. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  11. Demonstrated superior interpersonal and communication skills, both written and verbal.
  12. Effective time management and organizational skills with the ability to organize and prioritize as required Demonstrated discretion and maturity when handling confidential information.
  13. Demonstrated commitment to the safety of co-workers and patients.
