Social Worker Bachelors (NEO Navigation)

KEY FUNCTION: 

Support program development, provide clinical assessments, treatment planning, family interventions/ education, and referrals to community agencies to support patients and their families.

REPORTING:         

Under the general direction of the Coordinator, NEO Navigation.

DUTIES:

1. Provide client-centred social work assessment and treatment services to clients and family.
2. Develop appropriate treatment plans based on best practice treatment modalities.
3. Provide clients with a range of services including education, counselling, client advocacy, caregiver support, financial planning, referral to outside resources, and case management.    
4. Educate client and family in coping strategies, decision-making, utilization of personal and community resources, and make referrals to community resources as required.
5. Collaborate with clients, families, interprofessional team members, and community partners and advocate to ensure client needs are met.
6. Maintain clinical records of treatment in accordance with professional standards of practice.
7. Maintain statistical data and participate in research projects as required.
8. Develop summary program statements and eligibility criteria for various treatments.
9. Participate in case conferences and medical rounds for multidisciplinary service collaboration.
10. Monitor and manage referrals and assist with wait list management.
11. Maintain an organized appointment schedule.
12. Maintain and adhere to HSN policies, procedures and provincial social work standards.
13. Provide individual and systemic advocacy to address individual and systemic discrimination.
14. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
15. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
16. Educate and promote health, safety and wellness in the work place.
17. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
18. Represent the department or program on various committees and in meetings as required.
19. Perform other duties as required. 

QUALIFICATIONS

EDUCATION AND TRAINING: 

1. Minimum of a four (4) year Bachelor’s Degree in Social work is required.
2. Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
3. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

EXPERIENCE: 

1. Minimum of five (5) years’ related clinical experience.
2. Extensive experience working with children/families is required.
3. Experience working with diverse cultural populations is preferred.

KNOWLEDGE/SKILLS/ABILITIES: 

1. Demonstrated knowledge of the Regulated Health Professionals Act and Family and Child Legislation.
2. Demonstrated ability within therapeutic patient teaching and communication skills with adults and children.
3. Demonstrated ability to work as a member of a multidisciplinary team, as well as being able to work independently in a fast pace environment with challenging clients and families.
4. Demonstrated knowledge of available community support services for children and families.
5. Demonstrated willingness to learn and work in a non-judgmental capacity.
6. Demonstrated cultural sensitivity and experience working with diverse cultural populations.
7. Demonstrated experience working with children and their families coping with developmental delays and/or mental health issues.
8. Demonstrated training, experience or utilization of lean methodology for process improvement.
9. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
10. Demonstrated superior interpersonal and communication skills, both written and verbal.
11. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
12. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
13. Demonstrated discretion and maturity when handling confidential information.
14. Demonstrated commitment to the safety of co-workers and patients.
