KEY FUNCTION:
Provide practical and psychosocial support, working from a strength-based and trauma-informed approach, assisting clients to maintain safe, stable housing while building life skills and community connections.
REPORTING:
Under the general direction of the Clinical Manager, Lotus Program.
DUTIES:
1. Work in collaboration with the interdisciplinary team to provide psychosocial support to clients in a transitional, supportive residential living environment.
2. In collaboration with the MSW, develop goals of care and implement treatment plans.
3. Work collaboratively with community providers and provide advocacy for clients as required.
4. Provide psychosocial education to clients and/or caregivers through individual and group support.
5. Assist clients with navigating income supports, ID replacement, and community resources.
6. Provide support through check-ins and home visits post discharge, working with community providers to ensure seamless transitions.
7. Document assessments, intervention plans, progress notes and outcomes in a timely manner as per program policy standards.
8. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
9. Provide guidance, training and orientation for new staff and students.
10. Establish and maintain pathways to refer clients to on-going services.
11. Apply the Code of Ethics and Standards of Practice as outlined by the Ontario College of Social Workers and Social Service Workers.
12. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
13. Participate in the accreditation process by engaging in best practices, audits, assessments and improvement initiatives.
14. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
15. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
16. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
17. Educate and promote health, safety and wellness in the workplace.
18. Represent the department or program on various committees and in meetings as required.
19. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a Bachelor’s Degree in Social Work from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
3. Certification in one or more structured psychotherapy modalities (e.g., Cognitive Behavioural Therapy (CBT), Motivational Interviewing (MI), Dialectical Behavioural Therapy (DBT)) is preferred.
4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of three (3) years’ full-time equivalent experience working in a Mental Health and Addictions setting.
2. Experience in community-based, out-patient programs is preferred.
3. Experience providing individual and group setting treatment is preferred.
4. Experience in program development and providing education is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of addictions related care (i.e. withdrawal symptoms, withdrawal management, and medication treatment available).
2. Demonstrated knowledge of evidence based practices of recovery principles and stages of change.
3. Demonstrated ability to complete psychosocial assessments, treatment planning, and referrals.
4. Demonstrated ability to practice in the areas of clinical counseling with adults and families; strong client and family service orientation.
5. Demonstrated health care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
6. Experience providing both individual and group interventions.
7. Demonstrated excellent judgment, critical thinking and proven analytical skills.
8. Demonstrated knowledge of how to apply harm reduction practices.
9. Demonstrated knowledge of culturally safe practices.
10. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
11. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
12. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
13. Demonstrated superior interpersonal and communication skills, both written and verbal.
14. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
15. Demonstrated discretion and maturity when handling confidential information.
16. Demonstrated commitment to the safety of co-workers and patients.
