KEY FUNCTION:
To provide assertive Community Outreach and actively participate on a multidisciplinary team to provide direct client services for people who are living with serious, complex mental illness, and who require long-term support.
REPORTING:
Under the general direction of the Clinical Manager, Psychosocial Rehabilitation.
DUTIES:
1. Provide evidence-based treatment for individuals in the Psychosocial Rehabilitation programs.
2. Complete intake assessments.
3. Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.
4. Document assessments, treatment plans, progress notes, and outcomes in a timely manner.
5. Assist in and act as a resource for client and family education.
6. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.
7. Provide guidance, training and orientation for new staff and students.
8. Work collaboratively with community providers to provide wrap around care for complex clients.
9. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
10. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
11. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
12. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
13. Educate and promote health, safety and wellness in the work place.
14. Represent the department or program on various committees and in meetings as required.
15. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a four (4) year Bachelor’s Degree in Social Work, from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
3. Certification in Cognitive Behavioral Therapy, including forty (40) hours of theory and supervision hours, is preferred.
4. Additional training in Addictions and Counselling and Treatment is preferred.
5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum three (3) years of recent, relevant experience in providing psychiatric assessment, treatment, monitoring and evaluation of patients in a community setting.
2. Minimum of one (1) year recent experience managing community treatment orders (CTO).
3. Experience in community-based, outpatient programs is required.
4. Experience providing treatment in a group setting is preferred.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated ability to practice in the areas of Psychotherapies with adults and families.
2. Demonstrated Health Care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
3. Demonstrated skills in Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and individual and group interventions.
4. Demonstrated knowledge and understanding of principles of recovery as it applies to mental health and addictions treatment.
5. Demonstrated ability to manage rapidly changing mental health states and psychiatric emergencies.
6. Demonstrated skills in a range of individual and group psychotherapies, as well as experiential therapies for the treatment of addictions & mental illness.
7. Demonstrated excellent judgment and proven analytical skills.
8. Demonstrate strong client and family service orientation.
9. Demonstrated knowledge of culturally safe practices.
10. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
11. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
12. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
13. Demonstrated superior interpersonal and communication skills, both written and verbal.
14. Effective time management and organizational skills with the ability to organize and prioritize as required.
15. Demonstrated discretion and maturity when handling confidential information.
16. Demonstrated commitment to the safety of co-workers and patients.
