KEY FUNCTION:
Provide best practice assessment, treatment and support to individuals and families with substance use, problem gambling and/or concurrent disorders.
REPORTING:
Under the general direction of the Clinical Manager, Regional Outpatient Addictions Services.
DUTIES:
1. Provide evidence based substance use/gambling treatment for youth, adults and families individually or in a group setting.
2. Complete program intakes and develop/implement treatment plans with clients and families accessing services.
3. Complete addictions assessments for treatment planning purposes on individuals as required.
4. Document assessments, intervention plans, progress notes and outcomes in a timely manner (as per MHAP policy standards).
5. Assist in and act as a resource for client and family education.
6. Consult and liaise with appropriate resources to ensure quality care and maintain client safety.
7. Provide guidance, training and orientation for new staff and students.
8. Work collaboratively with community providers to provide wrap around care for complex clients.
9. Evaluate client care and satisfaction on an ongoing basis and formulate solutions for improvement.
10. Participate in program development and facilitate a variety of groups.
11. Participate in the accreditation process by engaging in best practices, audits, assessments and improvement initiatives.
12. Help clients identify and use health resources, including those available in the community and through agencies.
13. Maintain up-to-date charting/records in compliance with HSN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
14. Participate in research, program development and evaluation, and quality assurance activities.
15. Provide short-term counselling services, with particular regard to individual/family issues and decisions that impact the health of the client/family.
16. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
17. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
18. Represent the department or program on various committees and in meetings as required.
19. Educate and promote health, safety and wellness in the work place.
20. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a Master’s Degree in Social Work from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
3. Current Cognitive Behavioural Therapy (CBT) Certification is preferred.
4. Current Certification of Canadian Problem Gambling is preferred.
5. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of three (3) years’ experience working in an addictions and mental health setting.
2. Experience in community-based, out-patient programs is required.
3. Experience facilitating treatment groups is required
4. Experience providing individual treatment is required.
KNOWLEDGE/SKILLS/ABILITIES:
1. Demonstrated knowledge of evidence based practices of recovery principles.
2. Demonstrated ability to complete addictions assessments, treatment planning, and referrals.
3. Demonstrated ability to practice in the areas of clinical counseling with adults and families.
4. Demonstrated health care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV).
5. Demonstrated ability to provide complex therapeutic interventions to clients such as Cognitive Behavioural Therapy (CBT), Dialectical Behavioural Therapy (DBT) and individual and group interventions.
6. Demonstrated excellent judgment and proven analytical skills.
7. Demonstrate strong client and family service orientation.
8. Demonstrated knowledge of culturally safe practices.
9. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
10. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
11. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and client information systems.
12. Demonstrated superior interpersonal and communication skills, both written and verbal.
13. Effective time management and organizational skills with the ability to organize and prioritize as required.
14. Demonstrated discretion and maturity when handling confidential information.
15. Demonstrated commitment to the safety of co-workers and clients.
PERSONAL SUITABILITY:
1. Successful criminal reference check is required for vulnerable sector.
2. Ability to use tact and discretion in dealing with health care providers and clients.
3. Demonstrated ability to work effectively as a member of an interdisciplinary team.
4. Demonstrated ability to perform with minimal supervision; to prioritize duties.
5. Valid driver’s license with access to reliable vehicle.
