KEY FUNCTION

Provide ongoing, assessment, education and treatment to clients with an eating or feeding disorder and their families, across the age span.

 

REPORTING   

Under the general direction of the Coordinator, Counselling and Treatment.

 

DUTIES:

  1. Complete intake assessments and attend client case conferences as required.
  2. Provide individual and group best-practice therapies to clients and families, with informed consent.
  3. Develop, coordinate, implement and evaluate treatment plans, in collaboration with the interdisciplinary team and in consultation with patients and families and other stakeholders, as required.
  4. Communicate pertinent information and recommendations to clients, the interdisciplinary team and other care providers as required.
  5. Assist clients in identifying and using health resources, including those available in the community.
  6. Participate in developing, planning, implementing and evaluating new therapeutic programs, based on clinical needs and best practices.
  7. Evaluate and develop safety plans with clients and families as appropriate.
  8. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  9. Maintain up-to-date documentation/records in compliance with HSN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
  10. Participate in research, program development and evaluation, and quality assurance activities.
  11. Actively pursue learning opportunities and communicate new developments and best practices through education sessions with team members.
  12. Assist in overcoming barriers within the health care system to ensure timely delivery of services.
  13. Liaise with community agencies and assist with making connections to community resources/supports as required.
  14. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
  15. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  16. Educate and promote health, safety and wellness in the work place.
  17. Provide supervision and mentoring to students.
  18. Represent the department or program on various committees and in meetings as required. 
  19. Perform other duties as required. 

 

QUALIFICATIONS

       

       EDUCATION AND TRAINING: 

  1. Minimum of a Master’s Degree in Social Work from an accredited University.
  2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
  3. Certification in Cognitive Behavioural Therapy (CBT), Family-Based Therapy (FBT), Motivational Interviewing (MI) and Dialectical Behaviour Therapy (DBT).
  4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

 

       EXPERIENCE: 

  1. Minimum of three (3) years recent and relevant experience in the field of Mental Health and Addictions with preference for Eating Disorders experience.
  2. Experience working with families.
  3. Experience working in a community-based, out-patient program or regional program preferred.
  4. Experience providing group and individual treatment and counselling, across the age span.
  5. Experience in public education, group presentations and teaching, and community mental health service delivery preferred.
  6. Experience with information technology (video conferencing, electronic mail, telehealth, website development) for consultation, education, and treatment purposes.
  7. Experience in program development and process improvement.

 

       KNOWLEDGE/SKILLS/ABILITIES: 

  1. Demonstrated knowledge of the principles of recovery as it applies to eating disorders treatment.
  2. Demonstrated knowledge of best practices as it relates to eating disorder treatment and levels of care.
  3. Demonstrated knowledge of the principles of pediatric, adolescent and adult learning theory.
  4. Demonstrated in-depth knowledge of psychological factors affecting eating disorder clients and in-depth understanding of the psychosocial, physical and functional problems endemic to eating disorders.
  5. Demonstrated knowledge of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV).
  6. Demonstrated critical thinking skills and excellent presentation skill.
  7. Demonstrated skills in delivering education and treatment in a group setting.
  8. Demonstrated strength in program planning, development and evaluation.
  9. Demonstrated leadership and problem-solving skills.
  10. Demonstrated excellent judgment and proven analytical skills.
  11. Demonstrated ability to liaise with other health care professionals, programs, and external agencies.
  12. Demonstrated knowledge of Indigenous and Francophone culture.
  13. Demonstrated knowledge and use of the Ontario Telemedicine Network (OTN; OTN-PCVC) and willingness to use in clinical practice.
  14. Demonstrated ability to mentor students and support personnel.
  15. Demonstrated training, experience or utilization of lean methodology for process improvement.
  16. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  17. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  18. Demonstrated superior interpersonal and communication skills, both written and verbal.
  19. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  20. Demonstrated discretion and maturity when handling confidential information and sensitive issues.
  21. Demonstrated commitment to the safety of co-workers and patients.

     

     PERSONAL SUITABILITY:
