KEY FUNCTION:
Provide psychosocial support to children and families managing chronic conditions (i.e. Cystic Fibrosis (CF), gender diversity, and diabetes). Conduct one-on-one counseling sessions, address emotional and behavioral needs, and connect patients and families with community resources and support systems. Collaborate closely with interdisciplinary medical teams to ensure comprehensive, patient-centered care, while also advocating for the social and emotional well- being of pediatric patients throughout their treatment journey.
REPORTING:
Under the general direction of the Clinical Manager, Pediatric Outpatient Services.
DUTIES:
1. Provide comprehensive psychosocial support to patients and their families managing chronic childhood diseases (i.e. diabetes and cystic fibrosis).
2. Provide individual support to pediatric patients and families, with respect to acceptance, psycho-education, and skill building in areas related to managing chronic childhood diseases.
3. Assist in identifying emotional and psychological stress related to managing diabetes (i.e. feelings of burnout, anxiety, or frustration) by administering and supporting Diabetes Distress Screening.
4. Provide affirming, non-judgmental support to individuals exploring or expressing their gender identity, offering counseling to help clients and families navigate challenges related to gender identity, including social, emotional, and mental health concerns.
5. Develop and support adult transition plans in collaboration with patients, families, stakeholders, and the interdisciplinary team, based on identified needs.
6. Provide intake assessments and attend client case conferences as required.
7. Assist patients and their families in accessing financial assistance to manage the costs associated with medical care by connecting them to non-profit organizations or community-based assistance programs.
8. Maintain up-to-date charting/records in compliance with HSN, departmental policies and procedures, and the Ontario College of Social Workers and Social Service Workers standards of practice.
9. Participate in developing, planning, and implementing new therapeutic programs based on clinical needs.
10. Participate in research, program development and evaluation, and quality assurance activities.
11. Actively pursue learning opportunities and communicate new developments and best practices through education sessions with team members.
12. Help patients identify and use health resources within the school system.
13. Liaise with community agencies and assist with making connections to community resources/supports as required.
14. Collaborate with members of an interdisciplinary health team to plan, implement, coordinate, and evaluate patient care in consultation with patients and families.
15. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
16. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
17. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
18. Educate and promote health, safety and wellness in the work place.
19. Represent the department or program on various committees and in meetings as required.
20. Perform other duties as required.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Minimum of a Master's Degree in Social Work from an accredited university.
2. Current Certificate of Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
3. Successful completion of the Non-Violent Crisis Intervention course is required.
4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
EXPERIENCE:
1. Minimum of two (2) years’ relevant clinical experience working with a pediatric population.
2. Clinical experience in Diabetes, Gender Diversity and Cystsic Fibrosis within a health care environment is preferred.
3. Experience in individual counselling is preferred.

1. Demonstrated knowledge and understanding of the Ontario Hospital Association (OHA)/Ontario Medical Association’s (OMA) Communicable Disease Surveillance Protocols.

5. Demonstrated knowledge of Indigenous and Francophone culture.
6. Demonstrated knowledge and use of the Ontario Telemedicine Network (OTN; OTN-PCVC) and willingness to use in clinical practice.
7. Demonstrated ability to mentor students and support personnel.
8. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.
9. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
10. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
11. Demonstrated superior interpersonal and communication skills, both written and verbal.
12. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
13. Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
14. Demonstrated discretion and maturity when handling confidential information.
15. Demonstrated commitment to the safety of co-workers and patients.
